Diagnose and treat acute and chronic illnesses, including diabetes, hypertension, CAD and vascular diseases. Develop and implement personalized treatment plans for elderly patients with complex medical conditions. Provide preventive care, including vaccinations, screenings, and lifestyle counseling. Coordinate care with specialists, physical therapists, and social workers as needed. Educate patients and caregivers on disease management and health maintenance. Qualifications: Graduate from an accredited Physician Assistant program or Nurse Practitioner program Licensed as a Physician Assistant (PA-C preferred) and Nurse Practitioner in North Carolina. At least 1 year of experience in Cardiolgy (new graduates with a strong interest in geriatrics are encouraged to apply). Proficiency in managing chronic conditions and coordinating care for geriatric patients. Familiarity with EMR systems and ICD-10 coding. Excellent communication and interpersonal skills.
