PRIMARY FUNCTIONS: Neighborhood Health is a community health center with multiple clinics in Northern Virginia that provides services to a predominately low-income and underserved patient populations. The Weekend Part-Time Pharmacist will assure the availability, quality, continuity, and appropriateness of pharmacy services for our patient population; assure the management of Neighborhood Health pharmacy activities within a pharmacy in accordance with the laws of the Commonwealth of Virginia, Board of Pharmacy regulations, and the Drug Control Act. All Neighborhood Health Pharmacy staff will abide by the standards of accreditation in Mail Order Pharmacy and Specialty Pharmacy. DUTIES/ RESPONSIBILITIES: (Essential Functions: Duties and responsibilities noted with bullets are considered to be the essential functions of the job) * Must be willing to assume the duties of Weekend Part-Time Pharmacist. * Provides professional pharmacy services as appropriate and pharmaceutical consultation to staff physician and nurses regarding appropriate therapeutic selection. * Verify that every pharmacist working at your pharmacy holds a current license to practice pharmacy. * Verify that every pharmacy technician working at your pharmacy holds a current registration, or that there is documentation on site showing enrollment in a Board approved training program for not more than 9 months. * Be responsible for ensuring that the practice of pharmacy is in overall compliance with laws and regulations. * Assists in supervision of all pharmacy activities and maintains records as specified by the Drug Control Act. * Provide safeguards against diversion of all controlled substances and drugs. * Ensure the annual inventory of all drugs in Schedules II, III, IV, and V, to include any expired drugs in Schedules II-V, is performed. * Ensure the pharmacy complies each month with the perpetual inventory requirement of Schedule II drugs found in Regulation 18VAC110-20-240. * Notify the Board of any theft or unusual losses of drugs as soon as discovered. * Ensure all security measures are in compliance and operational. * Maintains and uses up-to-date patient drug profiles. * Assists in maintaining an up-to-date formulary; uses the formulary to dispense the proper generic or brand-name drug as prescribed by Neighborhood Health provider staff. * Assists in selection and control of all pharmacy drug stocks and supplies and maintains inventory. * Assists in the administration of pharmacy audit procedures. * Maintain pharmacy license in good standing. * Performs necessary compounding, using appropriate supplies, equipment, and ingredients. * Assists in development of appropriate clinical pharmacy goals and objectives. * Provides patient medication counseling and education through the proper use of labels, accessory labels, and personal contact. * Ability to supervise and coordinate the activities of staff of pharmacists and support staff. * Provide pharmacy technician Board approved training program. * Foster a work environment that reflects teamwork. About the Job We have an opportunity available for a patient-focused Clinical Pharmacy Specialist I to join our Palliative Care team at MedStar Washington Hospital Center. Under limited supervision provides a higher level of direct patient care pharmacy services for an assigned specialized patient population to assure adequate patient care according to professional standards and practices. Advises/consults with patients and their families and with the medical and nursing staff. Participates in research and drug use outcomes management programs within specialty area; instructs and trains pharmacy residents and students. Acts as a mentor to clinical pharmacists. Primary Duties and Responsibilities * Evaluates therapeutic options and provides clinical guidance to staff clinical level I and II pharmacists residents and residents. Monitors drug therapy and develops patient care plans of assigned patients. * Evaluates medication orders for accuracy and thoroughness. Resolves discrepancies of medication orders through communication with the physician clinical pharmacist or nursing staff and dispenses medication. Enters and verifies computer entries and data for drug orders as needed. * Provides functional supervision for support personnel. Monitors attendance and work quality organizes and prioritizes resources based on workload and productivity and directs the workflow of support personnel. Ensures departmental standards and time frames are met. * Participates autonomously in-patient care rounds. Reviews patient specific data to evaluate patient's status and medication needs and develops a therapeutic plan. Counsels/educates patient on their medications e.g. to correct dosing schedule and duplicate therapy. * Responds to evaluative and/or non-evaluative drug information requests. * Assures compliance of prescribers with clinical practice guidelines and other drug policies or third-party prescription benefit management procedures minimizes non-formulary drug use eliminates therapeutic duplication. Makes recommendations for appropriate alternative medications duplicate therapy and dosage changes. * Takes a leadership role in continuous quality improvement efforts and represents the department as an interdisciplinary interface in the assigned specialty areas. * Participates in the development of drug and disease state-based clinical practice guidelines that also influence clinical practice. This responsibility includes providing biomedical literature evaluations to various other committees as needed. These evaluations often require detailed analysis and evaluation of multiple references as well as the biomedical literature. * Writes reviews and presents high-quality evidence-based documents such as newsletters SBARs therapeutic interchanges drug monographs class reviews and drug shortage mitigation strategies among others ensuring time-sensitive completion and accuracy of clinical content. * Serves as a preceptor for pharmacy students and residents on clinical rotation. * Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate. * Coordinates supports and participates in research research protocol review and research development and publishes articles and presents at local and national meetings. * Provides educational services including professional development of pharmacy technicians and the clinical pharmacists pharmacy residents nurses prescriber and other healthcare professionals. * Contributes to the achievement of established goals and objectives and adheres to department policies procedures quality standards and safety standards and MedStar SPIRIT Values. Minimal Qualifications Education * Doctoral degree in Pharmacy (PharmD) or equivalent Pharmacy degree from an accredited college of Pharmacy required Experience * 5-7 years of hospital experience (with at least 4 years in a specialized area) required; or completion of PGY-1 and PGY-2 pharmacy residency required; or completion of a PGY-1 plus 2 years of direct patient care experience in a specialized area required Licenses and Certifications * PHARMD - Licensed Pharmacist in the District of Columbia required * Board certification in area of specialty within 1 Year required.
