1. Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience.

Job Summary
Cardinal Health manages the pharmacy at Lemuel Shattuck Hospital and we are currently recruiting for a Clinical Pharmacist. This is a 32 hour a week position that would consist of 4x8 hour shifts with rotating weekends. Hours of the facility are Monday-Friday, 7:30am - 10:00pm and weekends 8:00am - 4:30pm.
This pharmacist will work closely with medical staff to ensure safe, effect, and appropriate drug therapy. Along with supporting daily operational responsibilities that include, but are not limited to order verification, inventory management, and medication dispensing.
Responsibilities
* Reviews, processes and monitors patient medication orders to evaluate the appropriateness of use, dosage form, route of administration, regiment compliance, therapeutic duplication, drug interactions, adverse reactions and age appropriateness.
* Participate in multi-disciplinary rounds for a variety of patient populations.
* Checks technician prepared or prepares for dispensing all medications for patient specific use.
* Assists with drug procurement and inventory maintenance.
* Assists with precepting experiential education students from Schools of Pharmacy.
* Reviews non-formulary requests and prepares formulary reviews as needed for Pharmacy and Therapeutics Committee.
* Participates in performance improvement activities using evidence bases practice. Implements clinical initiatives (therapeutic interchange, renal dosing, ADR reporting) with regard to efficacy, safety, and cost effectiveness.
* Participates in Drug Utilization Evaluation program, Quality Improvement projects and prepares summary reports.
* Provide subject matter expertise to medical residents and medical staff.
* Performs other related duties as assigned or requested by the Director of Pharmacy.
* Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

Qualifications
* Bachelor's degree in Pharmacy or PharmD required.
* Must be a "good standing" licensed Pharmacist in the state of Massachusetts.
* Prior Hospital/Inpatient pharmacy experience preferred.
* Board certified in Pharmacology preferred.
* Completion of an ASHP-accredited PGY1 Pharmacy Residency preferred.
* Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals.
* Strong customer service skills.
* Flexibility to work various shifts and on-call as needed.
* May require vendor credentialing.

Job Summary
Under the direction of the Pharmacy Utilization Management (UM) Supervisor, the Clinical Pharmacist is responsible for reviewing, processing and managing the daily caseload of pharmacy prior authorization (PA) requests for coverage determinations (CD), and exceptions. Job Description
* Performs case reviews in automated prior authorization (Auto PA) tools and guides the case through the entire process to either approval or denial to ensure all coverage determinations and notices are consistent with applicable regulatory and accreditation requirements and turnaround times
* Provides all aspects of clinical support needed to perform drug prior authorization (PA) requests and consults as required with THP Medical Directors (MD) on cases that fail to meet coverage criteria
* Communicates daily with members, providers and other external customers regarding status and disposition of cases
* Oversees and addresses any quality issues with pharmacy technicians and confer with other clinical pharmacist on review questions
* Communicate with UM staff and providers when issues arise regarding policy interpretation, potential access availability or other quality assurance issues to ensure that members receive coverage determinations within timelines developed by all accrediting and regulatory guidelines
* Facilitates communication between Pharmacy UM department and other internal Tufts Health Plan departments by acting as a liaison or committee member on the development or implementation of new programs
* Performs other roles/duties and projects as assigned by the Pharmacy UM Supervisor or Manager
Salary Range
$107,989.60 -$161,984.40
