Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.

Position Overview:
Play a pivotal role in providing patient centered care by verifying patient specific prescriptions, validating compounded lots, and upholding compliance to USP guidelines. Ensure appropriate dosing and verify compounded medications while upholding quality control and compliance according to Strive Pharmacy Standard Operating Procedures (SOPs).
Key Responsibilities:
Follow proprietary formulations to compound commercially unavailable prescriptions into the appropriate dosage form
Performing calculations to ensure ingredients within a formula are measured and utilized properly
Inspecting lots and providing visual verification for the sterile laboratory through use of software and photos
Participate in regular audits and inspections
Ensure the compounding lab is cleaned and maintained in accordance with procedures
Maintenance, calibration and proper documentation of scales, electronic meters, etc.
Routinely checks for outdated or soon to expire chemicals
Coordinating with the Sterile Manager for personnel, compliance, and regulatory matters.
Developing and evaluating standard operating procedures for the compounding lab and formulations

Complete compounding continuous education responsibilities
Understand and be familiar with cGMP guidance (21 CFR 211), 503B Guidance, USP 795, USP 800, USP 795, and cGMP FDA 503B Guidelines, and other documents as assigned
Qualifications:
Minimum of Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) Degree
Active FL Pharmacist license in good standing
One year of sterile compounding experience is preferred
Knowledge of USP 795, USP 800, USP 795 (cGMP FDA 503B Guidelines preferred)
Understanding of medical terminology and pharmacy calculations

Standards for accuracy and potency must be consistently met

The Pharmacist will be responsible for overseeing the daily operations of the pharmacy department and is accountable for workflow management, clinical management of patients, continuous quality improvement, adherence to policy and procedure, compliance with internal and external regulatory bodies through the accurate filling of prescriptions, and ensuring the highest level of service to patients, providers, and pharmaceutical and payer partners. In addition to serving walk-in patients and employees, the Pharmacist will participate with other team members in the facilities setting to manage our patients' health.

Responsibilities include but are not limited to:
Compounds and dispenses prescribed drugs per physician's instructions in a hospital or healthcare facility setting.
Oversees non-sterile compounding processes and assists with related quality control and regulatory compliance.

**_Job Summary_**
Working in a hospital system pharmacy outsourced to Cardinal Health, the Pharmacist II adjusts and dispenses medication to patients and drives the compliant and efficient operations of the pharmacy. This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
**_Responsibilities_**
+ Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
+ Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
+ Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
+ Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
