General Purpose of Job: Responsible for the efficient and accurate preparation and dispensation of medications and other pharmaceuticals according to established operational and chemical procedures, and as prescribed by physicians. Compounded products are packaged, labeled and dispensed according to current laws and regulations. * Conducts orientation for new employees. * Handles consumer concerns, uses proper telephone etiquette, follows instructions, demonstrates initiative and dependability. * Exhibits self-confidence, resourcefulness, productive work habits and cooperation with others. * Performs clerical and housekeeping duties as needed. * Accurately measures substances. * Administer vaccines. * Able to process prescriptions for medications with risk evaluation and mitigation strategies (REMS) * Able to follow regulations on sales of ephedrine and pseudoephedrine products. Bargaining Unit: UUP Job Summary: The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking an hourly/per diem Outpatient Retail Staff Pharmacist. The Staff Pharmacist reports to the Associate Director of Retail Pharmacy Services and prepares, compounds and dispenses medications for outpatient and ambulatory care use as well as dispenses retail and specialty pharmacy related items. Additionally, the successful candidate will: * Provide information to health care practitioners to promote the rational use of medications. * Provide drug monitoring service. * Provide prescription medication counseling to patients as requested prior to discharge, time of dispensing and at patients bedside. * Participate in teaching programs for staff. * Monitor orders to ensure individualized dosage for both pediatric and adult patients. * Monitor serum levels and make individualized patient recommendations based on pharmacokinetic and pharmacodynamic properties. * Apply knowledge of pharmacology and pharmacokinetics to prevent duplicate therapy and drug interactions. * Provide direct supervision of medication preparation and dispensing; demonstrate an efficient use of resources (labor, services, supplies/materials, communication/data, and space). * Conduct mediation reconciliation and drug utilization review (DUR) to assure safety and appropriateness of medications. * Review patient profile to ensure appropriate drug selection and dosage (no drug duplication, interactions or allergies) and route of administration, with accurate and complete information. * Document all relevant interactions with patient and health care providers; document adverse drug reaction order clarifications per state and federal laws and regulations. * Efficiently perform pharmacist duties including the safe procurement, preparation, labeling, distribution and control of all drugs. * Participate in the pharmacy on-call rotation, as applicable. * Complete quality assurance functions, including quality improvement projects, as assigned; perform all other duties as assigned by management. * Contribute to the organization's overall process improvement activities; demonstrate critical thinking skills in identifying and analyzing problems and recommending solutions. * Promote a culture of safety as it relates to identifying safety and security risks proactively, and working to prevent injuries and/or harm to patients, visitors, or members of the SUNY Downstate community; observe the patient safety goals and commit to safe behavior and standards. * Oversee inventory management of both controlled and non-controlled substances, according to state and federal laws; maintain pharmacy records as required by law and institutional policy. * Participate in performance improvement activities; participate in and maintain training and education for competence required for the Staff Pharmacist position; complete all ongoing education and training as assigned by the department. * Interact with others in a positive and friendly manner; adhere to patient experience philosophy and customer service standards; represent SUNY Downstate in a positive manner, focusing on institutional core values, which drive innovation and discovery, respect of people, service to community; value integrity and embrace diversity and inclusion. * Demonstrate sensitivity to patient/family special needs, and be accountable for addressing each special need (unique personal history, physical limitations, belief system, language, communication style, literacy, health status, etc.). * Use ethical principles to guide decision and actions; recognize the value of each individual within a diverse and multicultural environment; treats others with respect and dignity. The pharmacist when covering a clinical role attends daily rounds to ensure and promote the safe, rational, efficacious, and cost-effective use of drug therapy. This will include activities such as review of medication orders, provide dosage adjustments per pharmacy policies for anticoagulation, pharmacokinetic antimicrobials, renally dosed medications, parenteral nutrition, and conduct daily profile reviews. The development of guidelines and policies, quality improvement processes, research, and assistance in the clinical training of staff, residents and students is also expected. This position offers the opportunity to serve as a preceptor for University of Michigan College of Pharmacy Introductory Pharmacy Practice Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs) and obtain adjunct faculty status within the College. General Purpose of Job: Responsible for the efficient and accurate preparation and dispensation of medications and other pharmaceuticals according to established operational and chemical procedures, and as prescribed by physicians. Completes the functions identified in the decentralized job descriptions and assist all associates with all other functions to assure complete, timely and accurate pharmacy services. * Compounding and supervising the compounding of Non-sterile medications up to and including Moderate Compounding as defined by USP and Sterile medications up to and including Medium Risk as defined by USP * Handle, dispense, verify, and manipulate hazardous medications as defined by USP to meet patient care needs. * Compounding caregivers adhere to the prescription or medical order for the strength, quality and purity of compounding preparations.
