Key Responsibilities
Patient Identification & Engagement
Identify patients on high-value therapies (e.g., GLP-1s, SGLT2 inhibitors, DOACs, insulins, Spiriva, Trelegy, HIV/HCV therapies) who are not filling at WFM pharmacies.
Conduct direct patient outreach via phone, text, MyChart, or in-person:
Explain the benefits of using WFM Pharmacy (affordability, delivery, integrated care).
Offer seamless prescription transfer support.
Address patient concerns related to pharmacy access, cost, or service.
Care Coordination & Service
Serve as a trusted liaison between patients, providers, and pharmacy staff.
Partner daily with the Community (Retail) Pharmacy team to:
