We care for patients with hyperglycemia on the cardiothoracic intensive care units and several other services and floors and manage their blood glucose until discharge, follow those patients by virtual visit several weeks after their discharge in our Hyperglycemia continuity clinic (CVC Metab Endo & Diab), answer pages initiated by patients after discharge if patients have concerns or questions regarding their glucose levels and insulin dosages, and conduct phone calls shortly after discharge for some patients.
