The Psychiatric Nurse Practitioner will provide outpatient psychiatric evaluation and medication management services at HCHN. Although a contracted position, providers must understand and support the vision and mission of HCHN. Understands that employment is contingent upon adherence to HCHN policies and procedures, TJC, OSHA, and HIPAA rules and regulations - related codes and laws. General Responsibilities * Performs medical examinations, diagnoses illnesses, and treats patients in accordance with their medical and mental health needs. * Works in collaboration with behavioral health staff to coordinate client care, and assist with insurance pre-authorization and caseload management. * Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders. * Prescribes, recommends or discontinues drugs and treatments to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement. * Collaborates with counselors, social workers, or other treatment team members to coordinate client care. * Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries. * Counsels clients during appointments, as necessary. * Designs/participate in the development of individualized treatment plans, incorporating best and leading practices into treatment interventions. * Examines or orders laboratory or diagnostic tests for clients to provide information on general physical conditions and behavioral health disorders. * Collaborate with guardians, relatives, and significant others on clients conditions and treatment when clinically appropriate * Prepares and submits assessments, progress notes, reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required * Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions * Follows all procedures and protocols as per HCHN policy, federal and state regulations, and standard practice, and in accordance with the Collaborative Practice Agreement. * Advised patients on good health practices. * Ensures patients receive educational materials that pertain to their patient's health needs. * Regularly advises the management team on issues regarding operations and patient care including staffing, data collection and interpretation clinic flow, appointment systems, client recall process, ancillary services, and customer satisfaction. * Participates in the effectiveness review of the clinical operation activities which includes but is not limited to, patient flow, laboratory services, x-ray services, pharmacy services, referrals, and all other activities within the clinics. * Participates in management and clinical reviews directed by BHCDA, PHS Region V., State of Michigan FIA, Michigan Department of Community Health, and any other applicable Federal, State, or Local regulatory agencies. * In accordance with grant requirements, adheres with the HCHN Health Care Plan as evidenced by compliance with the standards within the plan. * Participates in community and organizational activities designed to modify community health-seeking behavior, epidemiology, and community health care needs. * Active participant in HCHNs Performance Improvement Process, Providers meeting, and other committees as scheduled. * Other duties as assigned.
