The Clinical Dietitian provides overall nutrition care to patients and is 40 hours per week in an Onsite format. Assures that the nutritional needs of the individual patients are being met and that all documentation as it relates to the patient?s nutritional status is complete and accurate. Plans and administers activities related to resident/patient nutrition care, complying with the standards established by Aramark SeniorLIFE+, regulatory agencies, and clients. Establishes and maintains effective multidisciplinary relationships to provide a unified approach to resident care.
Job Responsibilities
Maintains dietetic registration, government-required credentials and/or licensure and continuing education requirements
Conducts patient nutrition assessment on a population of varying levels of acuity patients
Scope of practice may include residents and short term SNF interaction
Manages the development and implementation plan for nutrition intervention in accordance with the Nutrition Care Process
Conducts monthly tray audits and monthly Steritech kitchen audit
Handles food preferences and audits from Meal Tracker to PCC, along with all clinical charting
Provides nutrition counseling for individuals, which may include resident's family interaction
Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic concerns
Provides appropriate follow-ups in accordance with the patient?s treatment goals, referring patients for outpatient counseling, community, or home health services as appropriate
Delivers strong operational performance by executing in accordance with Aramark SeniorLIFE+ and regulatory agency standards and programs, continually monitoring operations, and completing assessments and necessary action plans to provide optimal nutrition care and meal service to patients/residents
Works cooperatively with medical staff as well as food and nutrition services staff to ensure conformance to prescribed nutrition care orders
Provides education that ensures compliance with food safety, sanitation, and overall workplace safety standards
Provides appropriate and timely documentation that summarizes the nutrition care plan in a patient?s medical record, including nutrition assessment, diagnosis, plan, implementation, and progress towards goals
Maintains clinical productivity standards and accurate record keeping of daily clinical activities, including information for scheduling and billing
Demonstrates accountability for the proper use of patients? protected health information

Job Specific Position Duties: As part of this role, your responsibilities encompass, but are not limited to:
Administering comprehensive nutrition prevention programs, including nutrition assessment, prescriptions, coaching, counseling, and various other health and nutrition activities to cater to the needs of the installation community.
Collaborating with H2H & HP staff to deliver programs in diverse community settings, such as the MTF, community facilities, and individual unit worksites.
Executing nutrition policy and program requirements in accordance with instructions and guidance set by AF/SG.
Applying a wide range of dietetics and behavior change principles, practices, and procedures to plan, implement, and evaluate H2H and HP nutrition programs.
Assessing the nutrition behaviors, environment, and needs of the beneficiary population in partnership with H2H staff, utilizing relevant tools and determining nutrition priorities aligned with AF HP healthy weight and nutrition goals.
Employing evidence-based nutrition methods and behavior change theories in collaboration with H2H and HP staff to create, coordinate, implement, market, and evaluate comprehensive nutrition programs and services, as well as other health and wellness programs. These are aimed at enhancing nutrition, total force readiness, and disease prevention/intervention for the beneficiary population.
Continuously researching, developing/acquiring, and maintaining materials for the implementation of evidence-based nutrition programs and services that address population health priorities and beneficiary needs.
Providing Medical Nutrition Therapy (MNT) for MTF clients and, as mission allows, developing nutrition components for chronic disease management protocols.
Using adult learning and behavior change theories (e.g., Motivational Interviewing, coaching, counseling) and teaching techniques to conduct various individual and group nutrition and lifestyle interventions.
Ensuring data quality management (e.g., coding, MEPRS) and meeting reporting requirements in program execution as per AF instructions, guidance, Joint Commission, and Accreditation Association for Ambulatory Health Care standards.
Documenting nutrition education and/or medical nutrition therapy in compliance with AND Nutrition Care Process, AND Nutrition Care Manual, and other relevant standards in the electronic health record.
Offering expertise to develop, implement, and assess nutrition programs for diverse populations.
Communicating and coordinating nutrition health promotion programs among local leadership and targeted beneficiary populations.
Applying suitable social marketing tools to achieve program goals and objectives.
Actively participating in teleconferences, updates, working groups, review panels, and symposiums related to nutrition education/MNT programming, management, or operations.
Delivering briefings, educational sessions, seminars, and worksite wellness programs as required.
Maintaining positive relationships with both internal and external MHS customers.
Collaborating closely with the interdisciplinary team to identify target populations.
Minimum Qualifications (Applicant must have authorization to work in the USA):
Education: A bachelor's degree from a college or university accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) or completion of a Didactic Program in Dietetics (DPD) accredited by the ACEND, or completion of a post-Baccalaureate degree Dietetic Internship Program of supervised practice (for a minimum of 1200 hours) accredited by ACEND.
Experience: A minimum of three (3) years of experience in health promotion, obesity & weight management, and lifestyle counseling within the last 5 years. This experience should include knowledge of the Diabetes Prevention Program Outcomes studies and the National Diabetes Prevention Program. Candidates must have a track record of coordinating and facilitating group health education classes and activities, organizing and implementing outreach events, and working with both youth and adult populations.
Licensure/Registration: A requirement to maintain current credentials through ongoing education in dietetics and nutrition in compliance with the CDR of the ACEND requirements.
Clinical Certification: Eligibility for Certification in Health Education from the National Commission for Health Education Credentialing (NCHEC) is essential. NCHEC certification is preferred.
Life Support Certification: Must possess and maintain an AHA or ARC Basic Life Support (BLS) certification.
