What You'll Do:
Conducts nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions.
Utilizes motivational interview to identify desired goals and outcomes of nutrition care in collaboration with patient/client and health care professionals. Considers the variables that may influence the patient/client's food related behaviors including culture and social determinants of health.
Evaluates and provides evidence based nutrition recommendations to address patient/client medical needs. Understands and evaluates patient/client nutrition status and its relationship to complex disease states including diabetes, cardiovascular disease, chronic kidney disease, gastrointestinal disorders, cancer, food allergies and obesity.
Collaborates with physicians and other health care professionals to advocate for and implement optimal nutrition care.
Provides preventative services and counseling at individual patient/client visits and group meetings.
Serves as preceptor providing nutrition education and guidance to dietetic interns and students. Develops training materials and participates in mentoring of staff.
Assists in monitoring compliance to Yakima Valley Farm Workers Clinic (YVFWC) procedures, federal qualified health center quality metrics and/or state and federal WIC requirements.
Maintains awareness of community demographics and resources. Connects clients to resources.
Designs and implements outreach activities to raise community awareness of program services.
Completes documentation in the appropriate electronic medical record to document patient/client interaction.
Actively participates in assigned meetings to promote care team collaboration and ongoing learning.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications:
Master's Degree in Dietetics from a U.S. regionally accredited college, university, or foreign equivalent for new graduates after January 2024. Dietitians credentialed prior to Jan 2024 are allowed to be grandfathered in with a Bachelor's Degree.
Completion of ACEND (Accreditation Council for Education in Nutrition and Dietetics) accredited academic coursework in a Didactic Program in Dietetics.
A Master's Degree in Dietetics serves as two years of experience for job offer purposes.
One year's experience in clinic, primary care, outpatient or community setting preferred.
Registered Dietitian or registration eligible (must obtain RD credential within 180 days of employment).

Direct Patient Care

Attend Nutrition at risk meetings with IDT and make appropriate recommendations,
Proficiency with PCC preferred.
Quaterly kitchen inspection with Dietary Manager.
Work with facility IDT to provide excellent nutrition care for residents.

Are a registered dietician in the state applied.
Typical candidates have at least two years of experience in skilled nursing and one year in a supervisory role.

Essential Duties and Responsibilities
* Develop and implement nutritional care plans based on assessments and knowledge of chronic diseases
* Consult with providers regarding nutritional status of patients communicating all pertinent information
* Work with the patient care team on high concern areas including:
* Adult and childhood obesity (IBT)
* Pre-diabetic and diabetic diet management (DSME)
* Management of special medical populations such as gluten, Celiac, food allergies, bariatric (MNT)
* General patient nutrition for children and adults
* Infant feeding problems - exampled by helping with breastfeeding mothers and starting a pump loan program
* Provide group nutrition education classes
* Participate in developing and implementing protocols for nutrition component of clinical services
* Familiar with using electronic health record (Ex. Athena)
* Monitor, and where applicable, oversee relevant provider and care staff to ensure delivery of care
* Other duties as assigned

Essential Functions:

* Acts as an educational resource for health care team personnel including dietetic interns, patients, family members and community. Orients/instructs new professional, registered and assistive and ancillary personnel on job duties, assignments and clinical activities of the unit. Provides learning opportunities to meet the nutrition education needs of patients/family/significant others.
* Responsible for direct date collection and data assessment of the patient's nutritional condition and stability. Supervises indirect data collection of assigned personnel.

Educate patient and family/caregiver about renal diet and fluid management

Participate in Quality Assessment Performance Improvement process, documenting and attending meetings per clinic policy
Compile and create nutrition materials to promote adherence to renal diet guidelines
