RegisteredNurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Performs the duties of Home Care Coordinator on the IDT as needed / assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
**NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (must have within 90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and / or managed care preferred.
1 year experience providing care as an RN required.
Bilingual with Fluency in Hmong required.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Job Title & Role Description:
The Triage Nurse will serve in a direct care and patient advocacy role and will ensure interdisciplinary care is optimized toward targeted outcomes. The Triage will ensure the safe and effective transition of care for patients discharging from the emergency department. Through telephonic-based outreach, the Triage Nurse will contact the patient or caregiver within 48 hours of discharge to conduct a comprehensive discharge assessment, conduct medication reconciliation, provide patient or caregiver education, support post-discharge order fulfillment (i.e., Durable Medical Equipment, Home Health Care, etc.), assess for any potential barriers to a successful transition to home, and evaluate for additional care planning needs. Additionally, the Triage Nurse will confirm the patient has a provider appointment within 7-14 days of discharge, or sooner, depending on the post-discharge needs of the patient.
Real-time clinical triage will be conducted either telephonically from a patient directly calling UpwardHealth for clinical care, a field staff member reaching out for escalation needs, or a systematic alert based on documentation entered in the system requiring review and triage. For Remote Patient Monitoring (RPM) alerts, the Triage Nurse will take immediate action for abnormal or out-of-range patient results.

This position provides responsive clinical support and symptom management, helping maintain continuity and excellence of care across all settings.
This role is ideal for an experienced, independent hospice nurse who is passionate about providing compassionate, patient-centered care and can confidently manage on-call responsibilities.
PRIMARY RESPONSIBILITIES
Respond promptly to patient and family calls, triaging needs and providing compassionate, timely hospice care.
Perform comprehensive nursing assessments including physical, emotional, and psychosocial evaluations.
Assess, manage, and alleviate symptoms such as pain, shortness of breath, agitation, nausea, anxiety, and other distressing conditions.
Implement and adjust the plan of care in collaboration with the hospice physician and interdisciplinary team to meet changing patient needs.
Administer medications and perform prescribed treatments according to hospice protocols and physician orders.
Conduct medication reconciliation at each visit to ensure safety, accuracy, and appropriateness.
Provide education and guidance to families and caregivers on symptom management, medication administration, and end-of-life comfort measures.
Offer emotional and spiritual support to patients and families, maintaining sensitivity to cultural and personal values.
Coordinate with the interdisciplinary team (IDG), including social workers, chaplains, CNAs, and volunteers, to ensure seamless and holistic care.
Report and communicate changes in condition promptly to the hospice physician and leadership team.
Participate in on-call triage, facility coordination, and home visits as needed to maintain continuous care coverage.
Ensure that all clinical notes, assessments, interventions, medication updates, and communications are accurately documented in the EMR within 24 hours of the patient encounter.
Attend and actively contribute to IDG meetings, staff trainings, and performance improvement initiatives.
Uphold all regulatory, compliance, and safety standards in accordance with state and federal hospice guidelines.
QUALIFICATIONS
Current RegisteredNurse (RN) license in the State of California (required).
Minimum 1 year of hospice or palliative care experience (preferred).
CHPN certification preferred but not required.
Strong clinical assessment and critical thinking skills; able to make autonomous decisions in the field.
Excellent communication and empathy when supporting patients and families through end-of-life care.
Reliable transportation, valid driver's license, and proof of auto insurance (required).
Proficiency with EMR systems and electronic documentation.
Bilingual abilities in Spanish, Hmong, or other languages commonly spoken by our patient population are a plus.
WORK SCHEDULE
6:00 AM - 10:00 AM: In-person facility visits (on-site)
10:00 AM - 10:30 AM: Unpaid meal break
10:30 AM - 2:30 PM: Documentation and paperwork (office or home)
2:30 PM - 5:00 PM: On-call availability and response as needed
(clock in only if called)
Compensation: Guaranteed 8 hours/day, plus additional pay for any calls between 2:30 PM - 5:00 PM

JOB SUMMARY
The registerednurse (RN) plans, organizes and directs/coordinates home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
DUTIES & RESPONSIBILITIES
Patient Care
The RegisteredNurse (RN) develops and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s);
Initiates proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care;
Provides patient, caregiver, and family counseling;
Provides patient and caregiver education;
Prepares clinical notes;
Communicates with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current home health plan of care.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Communication
Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
Communicates with community health related persons to coordinate the care plan.
Additional Duties
Participating in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training.
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
Supervises Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed.
JOB REQUIREMENTS
Must have graduated from an approved school of nursing and be licensed in California.
Maintains a current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Self-directed and able to work with minimal supervision.
Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.
Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Pay Range: $50 - $60 per hour
