LSS Recovery Center Oconomowoc provides up to 30 beds for adult women in need of Medically Monitored Treatment and Transitional Residential Treatment. Services are gender specific and trauma informed.

Under the guidance of program and clinical supervisors, you will:
Provide addiction counseling and case management services in residential, outpatient, or community settings
Conduct assessments, diagnostics, screenings, and referrals using tools such as UPC, ASAM, AODA, SUDDs, and DSM/ICD-10 (with supervision as needed)
Offer Employment Specialist services when applicable
Maintain accurate and timely documentation in compliance with state, federal, and agency standards
Facilitate individual and group therapy sessions
Communicate effectively with internal teams and external partners
Participate in outreach and public education efforts
Coordinate comprehensive case management and connect clients with community resources
Engage in professional development through trainings, meetings, and clinical supervision
Ensure services meet all program, licensing, and contractual requirements
Build and maintain strong professional relationships
Serve as a positive role model for clients and families

Your Role

Description
The SocialWorker LSW reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
This role is a licensed, masters-prepared socialworker who is an experienced and independent generalist clinician responsible for complex psychosocial interventions and facilitating the plan of care through discharge planning and resource utilization for all patient populations throughout the Northwestern Medicine Lake Forest Hospital campus.
The SocialWorker LSW proactively collaborates with Patient Care staff, Medical Staff, and other ancillary departments integrating the Department's functions and services with other aspects of the patient care process. For each problem, issue, or concern, this individual develops observable and measurable goals and expected outcomes in the areas of problem resolution, utilization and resource management, and patient satisfaction. The SocialWorker LSW meets the professional exemption under FLSA.
Responsibilities:
* Develops individualized treatment plans for patients, in collaboration with the patient, their family and the health care team.
* Coordinates and leads care conferences on patients with length of stays greater than 7 days.
* Documents unusual occurrences and patient relations issues; makes appropriate referrals to risk management, infection control and quality departments.
* Assesses patient and family needs for appropriate discharge services.
* Coordinates transfer to appropriate level of care such as skilled care, rehabilitation, custodial and home health care for complex care.

Job Duties:
Provide services to crime victims/witnesses. Management of criminal juvenile court caseload, ensuring all victims/ witnesses are treated properly during course of case prosecution. Orientate clients to their rights, the criminal justice system, and referral resources.
Interpret to witnesses the importance of participation. Reassure and persuade reluctant witnesses to cooperate with prosecution. Identify and reduce barriers for cooperation. Refer witnesses when appropriate to Witness Protection Program to ensure their safety.

Mentor and train students, interns, and volunteers. Conduct community presentations and attend job fairs and other crime victim events. Assist volunteers in staffing the victim/witness waiting rooms.

Scheduled Hours: 37.5 hours a week
* Monday-Friday (8:00am-4:00pm)
* Maintains regular contact with an active caseload- most of which is conducted in client's homes and in the community.
* Provides direct services to clients including individual and group treatment. Provides Substance Abuse treatment for dual-diagnosed clients.
* Consult with the supervisor regarding the transfer/placement of a client to another level of service, as needed.
* Assists clients in obtaining public entitlements, housing, community resources, etc.
* Organizes and participates in treatment-related clinical conferences and staffings on assigned clients.
* Conducts crisis intervention/evaluation/referral to persons who present themselves at the program.
* Performs initial assessments of clients after they have been deemed appropriate for participation in the program.
* Provide education and direction to clients, family members, and/or significant others.
* Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards.
* Assists in facilitating transportation of clients.
* Upon hire must have certification as an Alcohol/Drug Abuse Counselor (CADC) by the Illinois Alcohol and Other Drug Abuse Professional Certification Board (I.A.O.D.A.P.C.A.).
* Associate's degree and five (5) years' experience in behavioral sciences, or
* Bachelor's degree and three (3) years' experience in behavioral sciences, or
* Bachelor's degree in a related field and one (1) year of experience in behavioral sciences, or
* Master's degree in behavioral sciences and no experience.
* Must possess a valid driver's license.

Job Duties Include:

Maintains and submits accurate and timely records according to agency and contract standards, to meet daily, weekly deadlines. *

Uses training received and follows all policies and procedures of the agency, contracted agencies and the IRSS department. *
Reads and responds to APC work email daily or AT LEAST every 48 hours. Returns all phone calls, to the person that called, in an expeditious manner. *
