Our Elizabeth Upjohn Community Healing Center is looking for a SocialWorker (LMSW) to join the Outpatient Adult Substance Abuse/Mental Health Services team!
Our mission is to provide the community with integrated healing services . These services include children and family support, mental health counseling, addiction treatment and recovery, education and prevention.

Job Description
Provide outpatient clinical services including psycho-social assessments, treatment planning, and discharge/aftercare planning activities and provision of individual, group, or family sessions to be scheduled based on client needs.
Responsibilities
Performing admission, assessment, and discharge functions.
Assessing mental health, substance use, and other status of clients.
Preparing client initial and master treatment plans as appropriate.
Conducting individual, group and/or family therapy as appropriate.
Conducting education and training regarding aspects of substance abuse and severe and persistent mental illness.
Preparing/submitting reports in accordance with state, agency, referral and CARF program requirements.
Obtaining/monitoring funding authorizations from the state and insurance companies.
Maintaining professional relationships with referral sources.
NOTE: This position will be responsible for running some evening groups.
Qualifications
Master's SocialWorker Clinical License required
Certified Advanced Addictions and Drug (CAADC) credential preferred
Experience providing Substance Abuse counseling to individuals and groups required
Experience with CMH population and required documentation and electronic record preferred
Desired Competencies
Strong clinical skills and ability to engage in positive therapeutic relationships required. Ability to engage and retain clients
Ability to handle stressful situations calmly and professionally
Excellent oral and written communications skills, ability to operate computer equipment
Knowledge of federal, state, and local laws, regulations, program and services as they relate to the provision of services
Working knowledge of the system of services needed to address the needs of people with substance abuse issues and the ability to coordinate care between these programs.
Clinical experience using evidence based practices for Mental Health and Substance abuse including, but not limited to, Dialectical Behavior Therapy, Motivational Interviewing, Stage of Change theory, and Cognitive Behavioral Therapy

To work collaboratively with members of an interdisciplinary team in order to assist patients and families with complex psychosocial needs. These interventions may include, but are not exclusive to: alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).
Essential Duties
Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan.
Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient's needs.
Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient's current condition, situational factors, transition of care needs and psychosocial imperatives.
Provides consultation and resources to members of the healthcare team.
Responsible for maintaining relevant and current knowledge of community resources.
Participates or assists in department, regional, or statewide stakeholder committees, projects etc., as assigned/requested.
Develops and maintains current knowledge of federal and state regulations as they pertain to role.
Provide short term therapeutic support as appropriate for setting and location.
Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
Performs other duties as assigned.
