As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required

Assess patients for substance use disorders and evaluate to determine the most appropriate level of care and treatment support needed.
Make referrals, when determined appropriate and support patient access to treatment and needed services.
Provide substance use counseling and interventions either individually or in group sessions to promote recovery and good behavioral and mental health.
Develop and implement patient treatment plans based on clinical experience and knowledge.

Using the evidence-based 30 Days to Family model and family search and engagement practices, the Specialist works to ensure timely compliance with the Fostering Connections to Success and Increasing Adoptions Act of 2008, Senate Bill 683, and other relevant Missouri statutes.
The Specialist immediately begins identifying and engaging non-offending parents and grandparents, followed by an extensive search to locate, contact, and explore up to 80 additional maternal and paternal relatives for potential placement and ongoing support. Because foster care placements can be fragile, the goal is to identify multiple relatives or kin who can provide placement options or be part of a robust support network. Every effort is made to keep siblings together, maintain the youth's school of origin, and preserve meaningful relationships with friends and supportive adults.
Once a placement provider is identified-whether prior to the Protective Custody Hearing or later-the Specialist offers targeted support to promote placement stability. This includes assessing youth and family needs, identifying family and community resources, removing barriers to relative placement, and developing a comprehensive plan that outlines formal and informal supports available to the family.

The Family Coach is responsible for delivering high quality services in, family engagement, health, and wellness through a program coaching model mandated by the Head Start Program Performance Standards (HSPPS). Ensure that service delivery is conducted in the home environment with families enrolled in the center base program model.
Scope of Work:
School Readiness/Inclusion
Engage enrolled families in discussions about educational program options for their child, plan for transition to kindergarten and/or other program enrollment and provide support to the family during the transition process.
May be asked to participate in IEP and IFSP meetings as needed
May be asked to participate and plan FSP meetings.
Family Engagement and Community Partnerships
Initiate collaborative partnerships with families to establish mutual trust and respect.
Implement, document and report on the recruitment, selection and enrollment of eligible children and their families.
Directly coach and support the parent in developing skills to have positive, meaningful interactions with their child.
Meet with families twice a year for in person goal sitting and assessments with families.
Directly provide education, coaching and support to parents/guardians in the areas of child development, family development, health, safety, nutrition, and mental health/wellness.
Provide resources and referrals as needed for individual families, connecting families in ways that will support their family goals.
Maintain professional relationship and/or contact with local service providers, organizations and/or agencies who provide resources to families.
Provide appropriate supports and resources for emotional well-being, nurturing, responsive caregiving, and family engagement to support positive child outcomes.
Coordinate with Home Based Family Coaches and Family Committee to plan Group Connections/Family Events for children and families based on Head Start Policy.

Documentation
Monitor and document attendance concerns and follow up with families.
Complete and maintain timely and accurate data entry and/or documentation as required.
Ensure the strategies, skills, support, and procedures are delivered in accordance with HSPPS, program policies, philosophies, and goals.
Professional Development
Participate in professional development as required.
May be asked to be advisor for FDC.
May be asked to support other staff as needed
Maintain confidentiality of family records and/or all program information.

EDUCATION AND EXPERIENCE REQUIREMENTS.
Minimum of Associates degree in social sciences, human services, or related field with a minimum of 9 social service credits; or 2 years work experience in social services may be substituted for associate degree.
At least 2 years documented work experience in family development, social work, human services, and/or psychology
Experience working with low-income families, child or family preferred.
FDC Credential (provided by the program) must be obtained within 18 months of hire.
OTHER REQUIRED CAPABILITIES AND QUALIFICATIONS:
Ability to problem solve, handle crisis, and work with families and children of various cultures and low-income backgrounds.
Ability to work primarily in the homes with families residing in high-risk communities.
Ability to establish positive working relationships with families, agency staff, and community members.
Physically able to stand for long periods of time as well as stooping, squatting, running, and lifting an estimated 50 lbs.

The therapist will be responsible for providing family therapy services that address severe behavioral issues for referred youth.
Qualifications
Education:
Master's degree in Psychology, Social Work, Counseling, or a related subject area
Bachelor's degree with substantial social service experience working with families and youth
Job Responsibilities
Conduct MST assessment including review of referral information, identifying and engaging key participants, identifying systemic strengths and weaknesses, and developing interventions to address the referral behaviors
Engage primary caregiver and other key participants in active change-oriented solutions
Conduct sessions/meetings in the youth's natural environment (homes, schools, attends court dates, community). Must have a car to get to all these meetings.
Maintain clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with the 9 MST Principles and the MST Analytic Process.
Ability to engage and collaborate with all relevant systems and key participants within each system, to maintain alignment throughout MST treatment.
Participate in all MST training, supervision and consultation activities.
Maintain caseloads of 5 families at a time.
Staff are available to families via phone 24 hours a day, 7 days a week, with exceptions to vacations and personal time off

This staff member will receive MST certification through a five-day training on the MST model and will receive additional training as required
