Assessments & Evaluations: Conduct compliant, formalized and dynamic assessments to determine educational eligibility/level of functioning. Complete observations and evaluations, and manage annual and triennial reviews. Develop and maintain compliant IEPs and participate in multi-disciplinary teams.

Perform evaluations and develop treatment plans of patients with speech, language, cognitive and swallowing disorders

Qualifications
What you will need:
Minimum of a Master's Degree from an accredited Speech Language Pathology program
Valid Speech Language Pathology license in the state(s) of practice
Current CPR Certification in accordance with state regulations
A minimum of one year's work experience as a Speech Language Pathologist
Demonstrates knowledge of rehabilitation techniques related to complex neurological injury preferred
Communicates effectively and professionally in verbal and written interactions
Ability to lift 50 pounds
Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
Duties require fine motor skills,visual acuity, and walking/ standing for extended periods

PRIMARY JOB DUTIES
* Provides speech-language pathology services in inpatient and outpatient settings during assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage.
* On physician's referral, evaluates patients, develops specific treatment plans and goals, administers procedures and modalities specific to the learning needs and age of the patient, and plans for discharge utilizing appropriate Speech-Language Pathology techniques.
* Acts as a case manager for assigned patients.
* Prepares and maintains accurate and current patient records in accordance with departmental policies and procedures.
* Re-evaluates and revises patient's treatment plan on a regular basis, with approval of physician as necessary.
* Provides patient, family/caregivers with age appropriate instruction regarding therapy procedures and home program, and provides additional instruction as indicated.
* Instructs the patient and family/caregiver in care and use of assistive and adaptive equipment.
* Communicates with rehabilitation team members, family, physicians and third party payers regarding patient progress and program changes as appropriate.
* Attends and/or participates inter/intradepartmental in-services and meetings as required and appropriate.
* Participates in short and long-term program development, including participation on committees as requested.
* Participates in student intern and/or volunteer program as appropriate.
* Provides supervision to CFY as appropriate.
* Maintains clean, safe and orderly treatment areas and equipment. Reports to supervisor any problems or suggestions regarding safety in the workplace.
* Assists with orientation and supervision of other health care professionals as appropriate.
* Establishes and maintains effective communication with physicians, co-workers, patients and other hospital personnel.
