Responsibilities
Conduct evaluations and develop individualized treatment plans for residents with speech, language, voice, cognitive-communication, and swallowing disorders.
Deliver evidence-based therapy to improve communication and swallowing function.
Collaborate with interdisciplinary team members (nursing, PT, OT, physicians) to ensure holistic patient care.
Document assessments, progress notes, and discharge summaries in compliance with facility and regulatory standards.
Educate residents, families, and staff on communication strategies and swallowing safety.
Qualifications
Master's degree in Speech Language Pathology from an accredited program.
Current Colorado SLP license
Strong clinical skills with a patient-centered approach.
Excellent communication and teamwork abilities.
Prior SNF experience preferred but not required.

Pay Range: $70,000 - $104,000 per year
Guarantee: Pay is bi-weekly, and we guarantee your full compensation for the first 30 days of employment to ensure a smooth and supported start
Key Responsibilities:
Conduct comprehensive evaluations and assessments of clients with various challenges
Develop and implement individualized treatment plans to address specific needs

Responsibilities
Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. At Enhabit Home Health & Hospice, an SLP may often work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson's disease and dementia.
Qualifications
