This role will design and develop Participant care plans based on speech language pathology assessments. Evaluates the effectiveness of treatment plans and modifies goals, as needed Communicate with staff regarding Participants' needs in areas of communication strategies, dietary texture modifications, safe swallowing practices, and cognitive support, as needed. Communicate with family members and caregivers to ensure that therapeutic recommendations-such as communication aids, diet modifications, and assistive technology-are clearly understood, feasible, and can be appropriately supported at home.
