A DAY IN THE LIFE OF A PHYSICAL THERAPIST
As a physical therapy professional in home health care, you play an essential role in providing our patients with high-quality services that are in accordance with their plan of care. You assist the physician in evaluating the level of functioning in each patient by providing initial and ongoing assessments. After making decisions based on assessments, you help develop or revise the plan of care, prepare clinical and progress notes, and consult with the family and other agency personnel. As a Physical Therapist, you'll contribute to our success in the following ways:
Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care
Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.
Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
Conducts therapy practice within defined standards of care.
Directs the assistance of support staff to achieve patients' functional goals.
Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community
Physically demanding, high stress environment
Full range of body motion including handling, lifting and transfer of patients
Potentially work irregular hours including call hours if applicable
What is Required?
Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
Current, unrestricted PT license valid for the state of work
Minimum one (1) year of experience in a healthcare facility
Able and willing to travel within branch/office coverage area.
Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
Must be able and willing to travel 50%
Able to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.

Required Skills:
Strong knowledge of diagnostic groups including orthopedic, arthritis, neurological conditions (e.g., CVA, TBI, degenerative neurological diseases, SCI)
Experience with patients facing generalized weakness from medical or surgical conditions such as DM, ESRD, amputation, COPD, CHF, CABG
Ability to develop and implement individualized interdisciplinary care plans tailored to adult and geriatric populations
Familiarity with IRF-PAI and CMS payment systems
Proficient in administering evaluation tools and utilizing assessment probes as needed
Basic computer skills for documentation and communication
Preferred Education and Experience:
MS in Physical Therapy or higher
Licensed or eligible for licensure in the State of Oklahoma
1-3 years of inpatient rehab or acute care experience preferred
Recent graduates who are in the process of licensure examination may be considered for provisional appointment

BLS certification required

Ability to work Monday through Friday, 8 am to 5 pm

Qualifications
A day in the life:
Conduct 1:1 ABA therapy sessions with early learners and young adults with Autism through teaching sessions in the home or community setting.
Implement behavioral intervention and individualized treatment plans for children between the ages of 2 and 21.
Participate in BCBA trainings, team meetings, client observations and parent training as needed.
Maintain open and direct communication with the BCBA and other RBT team members as needed relating to the delivery of treatment to our pediatric patients.
Receive Supervision from your BCBA at a minimum of 10% of services provided.
Maintain professional boundaries around our child patients and their families as indicated by the BACB.
Enter data and all trials utilized daily for each pediatric patient for all programs targeted.
Remain available for all supervision sessions, phone conferences, and team meetings required by the BCBA.
Other duties as assigned.
