Duties and Responsibilities:
Ensure all case records, correspondence, and conversations regarding clients remain confidential as required by HIPAA compliance regulations.
Complete releases and GJR intake forms upon initial contact with client/placement/other parties ensuring copies are saved in the clinical record.
Provide home-based services to clients in compliance with Service Standards published by the Indiana Department of Child Services for the service that they are providing.
Communicate initial client contact or lack thereof to referral source and Compliance Coordinator within 48 hours.
Document client contacts through up-to-date case notes that are maintained in Case Management Pro (electronic database), VPN client files and Kid Traks (if applicable).
Provide monthly Progress Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) as well as any other specific written or verbal information requested by referring agencies.
Provide Treatment Plans, Safety Plans, and other required clinical documents/resources/tools within 7-30 days of referral to referral agencies. Updated Treatment Plans are due no less than every 90 days to supervisor for review and sent to referral source by pre-arranged due dates.
If applicable, complete visitation/parenting time logs and send to referral sources within 48 hours of services rendered ensuring all required sections are compliant with current state content standards.
Oversee completion of program required assessments, including but not limited to, UCLA PTSD, NCFAS, IARCA packets upon admission, discharge, three month and six-month follow-up.
Record all billable/direct and non-billable/indirect time on a daily basis, utilizing the established database while ensuring an accurate timesheet is maintained.
Maintain client files ensuring all required documents are in place adhering to the GJR inIndiana file checklist for each respective service being rendered. Submit and correct file audit identified issues within 7 days of receiving feedback from audits.
Working Conditions:
This position is responsible for working at multiple locations across the State of Indiana included but not limited to client homes, schools, parks, probation, DCS.
Must be able to walk, bend and stoop.
Must be able to lift 10 pounds on a regular basis
Requirements:
Therapists must meet one of the following minimum qualifications:
Master's or doctorate degree with a current license issued by the Indiana Behavioral Health and Human Services Licensing Board as one of the following: 1) Social Worker; 2) Clinical Social Worker; 3) Marriage and Family Therapist; 4) Mental Health Counselor; 5) Marriage and Family Therapist Associate; and 6) Mental Health Counselor Associate.
Master's degree with a temporary permit issued by the Indiana Behavioral Health and Human Services Licensing Board as one of the following: 1) Social Worker; 2) Clinical Social Worker; 3) Marriage and Family Therapist; 4) Mental Health Counselor; 5) Marriage and Family Therapist Associate; and 6) Mental Health Counselor Associate within 90 days of providing services under this contract. (Please note: In order to obtain a temporary license, the individual must apply for the license and temporary permit as well as have 2 years related clinical experience, or b. completion of the DCS approved training program).
License is required within 90 days of employment. In order to obtain a temporary license, the individual must first register for the licensure exam and may have additional requirements for temporary licensure. Please contact the Indiana Professional Licensing Board for additional instructions for obtaining a temporary license.

The Family Preservation Therapist provides professional, evidence-based therapeutic services to children and families with the primary goal of strengthening family functioning and preserving the family unit whenever safely possible. Essential job functions:

Develop, implement, and monitor individualized treatment plans in collaboration with community providers, informal supports, internal teams, and families to advance treatment outcomes and ensure continuity of care within established time frames.
Manage and respond to client crises and participate in family safety assessments and safety plan development as clinically indicated.
Maintain timely, accurate, and thorough clinical documentation in compliance with accreditation standards and Department of Child Services requirements.
Participate in case management activities including child and family team meetings, court hearings, and treatment team case reviews, providing objective clinical input and recommendations.
Advocate for the best interests of children and families by identifying service gaps, connecting clients to appropriate community resources, and communicating progress to placing agencies.

Engage in ongoing professional development through clinical supervision, training activities, field observations, and performance feedback processes to ensure high-quality service delivery.
Work Schedule: This position requires, on average, 40 hours per week. Normal work hours are Monday through Friday from 8 am to 4:30 pm. However, travel, therapy sessions and other tasks will often extend beyond these hours. This position will require participation in an on-call rotation.
Physical Demands & Work Environment: The employee in this role will work in an office setting, client homes, and/or community settings as appropriate to service delivery.
