Essential Functions: Assist in the provision of individual, group, and family counseling to youth and their families within the home and communities of the clients. Provide crisis intervention counseling to families in crisis, as assigned. Assist in the completion of comprehensive assessments of families in crisis. Perform case management duties and is accessible ”on call” to assigned clients on a 24-hour basis. Provide direct support to client families in implementing their case plan through constructive, active participation in family life. Teach and assist parents in meeting the physical, mental, and emotional needs of their children. Maintain an active caseload as assigned. Assist in the coordination of other community-based services to advocate the needs of clients on the caseload. Assist in the provision of appropriate referrals for clients and assist in linkage to those referrals as necessary. Maintain professional communications and teamwork as appropriate contacts occur with the referral agency. Serve as advocate for family preservation/unity. Assist in the provision of follow-up monitoring and services to families on closed case status. Attend staff meetings, supervision sessions, and inter-agency staffing on clients as required. Compile required reports on families in caseload and submit to supervisor in a timely manner, including progress notes, reports to appropriate agencies and courts as the needs dictate. Adhere to professional standards and ethics. Serves as a referral liaison by presenting agency services to schools and community agencies and organizations. Perform other duties as assigned. Physical Requirements: Ability to sit for extended periods while conducting counseling sessions, completing documentation, and participating in meetings. Ability to stand, walk, bend, and occasionally lift up to 20 pounds for activities such as setting up group sessions or transporting materials. Adequate vision and hearing to observe client behavior and respond effectively during sessions. Manual dexterity for operating computers, phones, and other office equipment. Capacity to travel locally to attend home visits, community outreach events, or off-site meetings as needed. Ability to maintain energy and focus during high-stress or emotionally intense situations. Prevocational Services Prevocational Services are individually designed to prepare a youth (age 14 or older) to engage in paid work, volunteer work or career exploration. Prevocational Services are not job-specific, but rather are geared toward facilitating success in any work environment for youth whose disabilities do not permit them access to other prevocational services. The service will be reflected in youth's plan of care and must be directed to teaching skills rather than explicit employment objectives. The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services. TASKS AND RESPONSIBILITIES Assist Project Director and Behavioral Health Lead Navigator in SAMHSA annual reports, continuation applications, etc. for Behavioral Health Department. Assist Project Director and Behavioral Health Lead Navigator with integration of behavioral health services and communication between disciplines (psychiatrists, physicians, APRNs, LCSWs, LMHCs, etc.). Attend weekly staff meetings, Behavioral Health Department meetings, and BMC general staff meetings. Communicate effectively with supervisor regarding caseloads, issues regarding supervision, and leave requests. Conduct street and community outreach during working hours, including some nights and weekends as needed. Assist Project Director and Behavioral Health Lead Navigator in the creation, scheduling, and presentation of education sessions/groups for HIV, STD awareness, mental health, Life Skills, substance use, and other BMC services. Distribute preventive materials including safe sex kits, condoms, and flyers. Educate target population, newly diagnosed HIV persons, and others about BMC's available services, treatment options, and assist with linkages to services. Engage and educate individuals aged 13 and up in risk-reduction discussions. Ensure all certifications required for your position remain active and that updated copies are submitted for your employee file. Ensure data collection is completed in a timely, accurate and consistent manner, including HIV and STD testing, follow-up questionnaires, and others. Make home visits and reach out to individuals that are high risk, hard to reach, and/or present an adherence problem to treatment. Navigate coordination and expedite services and linkages to additional wrap around care. Perform behavioral screenings and assessments including GPRA, questionnaire data, demographic data collection instruments, pre-post training assessments, and surveys of partners and staff. Assist Project Director and Behavioral Health Lead Navigator in performing periodic review of performance measures to assess progress toward goals, inform program development, and enable fine-tuning of implementation strategies. Present at internal departmental meetings and educate BMC staff about programs and services Provide HIV, hepatitis, and STD testing and counseling. Provide program with community referrals, providing name and location of available van and outreach sites. Recruit participants for all Behavioral Health programs, HIV/hepatitis/STD testing, and prevention services. Work with the Marketing Department to create campaigns and projects for social media. Perform all other duties as assigned by Director of Behavioral Health and/or Behavioral Health Program Managers. REQUIREMENTS Minimum High School Diploma, or equivalent. Outreach experience. This individual will assist with complex cases and internal process development and interacts closely with the BH Medical Director and other Medical Management staff to direct appropriate utilization and data capture. The clinician will provide education to physicians and other members of the team on the issues related to utilization review, including inappropriate admissions and placements. The Utilization Management Behavioral Health Clinician is also responsible for managing continuity of care, ensuring smooth and safe transitions, provider and patient satisfaction, patient safety and appropriate length of stay. Serves as a liaison between the Medical Director, physicians and office staff in resolving prior authorization questions, issues and problems; communicates denial determinations to providers when indicated.
