Job Summary
We're seeking a strategic, communicative and compassionate physical therapist who will diagnose and treat individuals that have conditions that limit their ability to move and work to improve their quality of life by alleviating pain and improving their physical function. Our physical therapy treatment plans require flexibility, collaboration, out-of-the-box thinking, and problem solving to provide our patients/residents the best care that pushes them to the highest level of physical function. This may require you to lift and transfer your patient/resident as well as handle being on your feet every day.
Responsibilities
* Plans, organizes and delivers physical therapy programs that help patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities
* Documents patient's/resident's information for evaluation
* Educates patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home
* Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements
* Analyzes and records the therapeutic progress of patients/residents; recommends necessary adjustments
* Follows guidelines, regulations, standards and safety procedures in the therapy process
Qualifications
Appropriate education level required in accordance with state licensure.
Must be licensed in the state(s) of practice as a Physical Therapist.
When applicable, may require valid driver's license.
Obtains and subsequently maintains required department specific competencies and certifications. Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

DUTIES & ESSENTIAL JOB FUNCTIONS
Greet, welcome and assist clients/residents, vendors and guests, to provide welcoming and professional image of organization.
Monitor, dispense and re-order client medications to ensure receipt of correct dosage, timely receipt and availability.
Conduct room-to-room apartment checks, along with facility checks as required or scheduled, and complete appropriate documentation to ensure safety and security of residents, staff and facility.
Document information, observations, concerns, issues and events in appropriate venue to provide accurate information to staff and management.
Perform light maintenance and housekeeping duties assigned to provide clean and safe environment.
Perform breathalyzer and unobserved urine drug screens on clients as required and maintain appropriate documentation to ensure drug- and alcohol-free living environment.
Monitor and track clients exits and entries into facility to always ensure knowledge of client location and activities and that all areas are monitored appropriately.
Monitor medication to residents according to established protocol to ensure exact dosage.
Transport clients to appointments/meetings to attend community recovery groups.
Answer phones, greet public, and route inquiries to appropriate personnel while providing positive public relations and accurate responses to questions.
Understand and abide by all confidentiality laws to protect clients and client information.
Provide and maintain reliable and prompt attendance, with the ability to work the schedule dictated by business necessity to include afternoon and overnight shifts, call-ins, weekends and holidays.
Conduct fire drills as instructed across all shifts to ensure a smooth and efficient procedure, and also ensure timely and safe evacuation of all residents, as well as fill out drill forms and submit them to the Safety Officer or HR.
Search client property at admission and throughout their stay in services
Review facility cameras for safety
Clean and disinfect client rooms as they discharge
PERIODIC FUNCTIONS AND RESPONSIBILITIES
Refer maintenance and repair issues to maintenance department.
Cover the front office assistants phone as needed.

Provide care and close supervision of patients. Purpose of position: To assist with providing extra support for children in the classroom ages birth to five. This position will assist the Teacher in handling challenging behaviors, social skill needs, and providing cognitive supports.
Below are the essential duties of this position. Other duties may be assigned, as needed.
May be relocated within the SENDCAA Head Start or Early Head Start sites to best fit program needs.
Consistent attendance and reporting to work on time is essential for this position to maintain consistency and routine in the program's day-to-day functions.
Essential Duties and Responsibilities:
Program
Education staff are trained as mandated reporters and will report any observations of concern to Cass County Social Services
Responsible for following all Head Start and SENDCAA program policies and procedures including Standards of Conduct and Confidentiality.
Participates in training and meetings, including training days, site meetings, reflective supervision & others at the supervisor's request.
Education
Maintain working knowledge of Head Start Performance Standards and the Early Learning Outcomes Framework
Participates in coaching process as assigned to promote positive, effective learning environments for children
Ability to take on tasks such as leading small group activities, reading aloud, transitioning children from one activity to another, and leading large groups at the supervisor's request
Assist classroom staff by supervising children in the classroom/bathroom
Assists in educational and enrichment activities such as reading, painting, imaginary play, etc.
Assist in preparing students for nap time
Provide coverage when teachers are away from the classroom such as preparation time or time off
Actively engages with children by sitting on the floor playing with them, engages in conversation with them, and actively works to build a relationship with each child while maintaining awareness of classroom needs

Become aware of Conscious Discipline skills and use them to fidelity in the classrooms.
Will participate in monthly Reflective Supervision with Mental Health and Disability Coordinator or Site Supervisor
Become familiar with and implement a child's Behavior Support Plan appropriately
Collects progress notes per session while assisting in each assigned classroom or site
Classroom Organization
Assists in the upkeep and maintenance of the classroom equipment, materials, and supplies
Assists in maintaining a clean and safe classroom by following policies & procedures
Responsible for daily cleaning including wiping down surfaces, sweeping, disinfecting toys/areas, etc.
Assist Teacher and Teacher Assistant in day-to-day classroom planning, preparation, and activities
Respect each family's cultural, ethnic, and linguistic diversity
May be asked to attend home visits when other staff may not be available
Family
Health and Safety
Models/Guides good health practices, such as tooth brushing and hand washing, ensuring they are followed
Assists in implementing family-style meals in a pleasant environment including conversation & socialization. See Mealtime procedure for further instruction
Helps ensure no child will be left alone or unsupervised at any time

Talents, skills, and abilities:
