    **Job Summary and Responsibilities** Under the direct supervision of the agency Director, with indirect supervision from the Chaplain Coordinator, the Hospice Spiritual Care Provider is responsible for providing spiritual/bereavement care to hospice clients, families, staff and volunteers. **Essential Functions:** Documents assessment, planning and coordination of Spiritual Care of the patient/family: - Spiritual Assessment as documented in the electronic medical record. - Patient/family care plan reflecting individualized goals and interventions. - Spiritual assessment completed within five calendar days of admit. Meets Spiritual care needs of patient and family: - Visits made per frequency order and notes reflect ongoing assessment of spiritual needs. - Increased visits/contacts as patient status declines. - Provide spiritual/emotional support to patients and families. - Coordinates spiritual care with community clergy as directed by the plan of care.
