Responsibilities
Conduct comprehensive clinical assessments (including medical history, risk factors, and medication review) and develop person-centered care plans with SMART goals based on medical, behavioral, and social needs.
Provide medication reconciliation, health education, and condition-specific teaching to strengthen member understanding, self-management, and adherence.
Collaborate with Lead Care Managers, CHWs, Behavioral Health, Housing Navigation, providers, and community partners to coordinate services and resolve medical and social barriers.
Triage clinical concerns, identify red flags, and escalate appropriately to NP/MD partners; provide brief interventions within RN scope as needed.
Participate in interdisciplinary Systematic Case Reviews (SCR), IDT meetings, and case conferences, offering clinical recommendations and follow-up planning.
Coordinate transitions of care (TOC) by supporting post-hospital follow-ups, reconciling medications, scheduling timely appointments, and ensuring continuity.
Maintain accurate, timely, audit-ready documentation in ECW, Google Suite, and payer/health plan portals; ensure all clinical assessments, screenings, and care plans meet required timelines.
Engage members through relationship-based and trauma-informed approaches, building trust with individuals who may struggle with traditional healthcare systems.
Identify gaps in care, clinical risk, or environmental barriers and collaborate with care teams to implement timely interventions.
Support HHN's startup model by adapting to evolving workflows, contributing to clinical process improvements, and helping build scalable care coordination practices.
Skills Required

Key Responsibilities
Provide strategic leadership and daily oversight of all clinical teams, including RN Case Managers, LVNs, MSWs, Chaplains, CNAs, and IDG processes.
Ensure compliance with CMS, CDPH, Joint Commission (if applicable), and hospice Conditions of Participation.
Oversee patient care delivery for an ADC of 100+ patients, ensuring quality outcomes and timely interventions.
Lead IDG/IDT meetings, clinical audits, quality initiatives, and performance improvement activities (QAPI).
Maintain and enforce visit frequencies, care plans, documentation standards, and regulatory timelines (FTF, NOE/NOC, CTIs, recertifications).
Partner with Executive Director/Administrator to drive operational performance, patient outcomes, staffing efficiency, and fiscal stewardship.
Recruit, train, mentor, and evaluate clinical staff to uphold a high standard of service and professionalism.
Serve as the primary clinical liaison with physicians, hospitals, families, and community partners.

Active RN license in California (required).

Graduate Students
If youre a graduate student enrolled in a postgraduate nursing program in certain nursing specialties and opting to serve part-time as a Reserve Officer, you may qualify for up to $50,000 in nursing school loan repayment assistance.
Practicing Nurses

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. As a Navy Nurse, you could work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA or San Diego, CA. Or you could provide medical support aboard one of two dedicated hospital shipsthe USNS Comfort and USNS Mercy. Additional opportunities are available on surface ships, with aircraft squadrons or even with the Fleet Marine Force.
QUALIFICATIONS AND REQUIREMENTS
To become a Commissioned Officer in the Nurse Corps, qualifications include:
U.S. Citizen between the ages of 18 and 41
Currently licensed and practicing nursing in the U.S. (new graduates must obtain a license within one year of beginning Active Duty service)
In good standing (as a student or graduate) with a CCNE-accredited U.S. education program granting a
Bachelor of Science degree
General qualifications may vary depending upon whether youre currently serving, whether youve served before or whether youve never served before.
PART-TIME OPPORTUNITIES
There are part-time opportunities as a Reserve Navy Nurse. Serving part-time as a Navy Reserve Nurse, your duties will be carried out during your scheduled drilling and training periods. During monthly drilling, Nurse Corps Officers in the Navy Reserve typically work at a location close to their homes.
