Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Job Duties
STATEMENT OF ESSENTIAL JOB DUTIES
* Conducts investigations and evaluations of individuals for involuntary commitment and detains individuals when legal criteria are met and commitment is appropriate.
* Assesses behavioral health emergencies and provides crisis intervention for individuals experiencing a behavioral health crisis in the community.
* Works collaboratively with behavioral health agencies, psychiatrists, emergency departments, inpatient units, residential facilities and other referral sources to assure the best outcome for each individual in need of behavioral health services.
* Coordinates and facilitates arrangements necessary for the holding of probable cause hearings for patients hospitalized in our county in need of continued involuntary treatment; provides court testimony concerning patient's need for further involuntary treatment.
* Performs ongoing community regarding involuntary treatment.
* Contacts appropriate hospitals for admissions, provides screening information and arranges for transport as needed.
* Participates in the ITA Court process and provides relevant information to prosecuting attorneys and others authorized through the judicial process.
* Maintains necessary records and files as well as statistical information on all program activities.
* Performs related duties as required.
Minimum Qualifications
A master's degree in social work, clinical psychology, psychiatric nursing or related field; PLUS, two (2) years of experience in the direct treatment of mentally ill clients; OR, any equivalent combination of training and/or experience which provides the required knowledge and abilities. PLUS, a valid and current license as a Licensed Mental Health Counselor, Licensed Social Worker or Licensed Marriage and Family Therapist. Must pass job related tests.

The majority of the work involves site visits to residences, hospitals, mental health agencies and detention facilities. Some work is performed in the usual office environment. The employee may be required to work evenings, weekends and holidays to provide 24 hour a day, 7 day coverage.

What You'll Do:

What You Bring:
Academic training in the social service field and in chemical dependency treatment as required in accordance with the qualifications for a Department of Health licensed Chemical Dependency Professional as stipulated in the WAC.
The counselor is required to maintain this status and complete continuing education requirements throughout their employment. The Counselor must also provide the agency with ongoing documentation of their current Washington Department of Health C.D.P. license credential throughout their employment.
Minimum Two years of experience in the social service field and in chemical dependency treatment is required in accordance with the qualifications for a Department of Health licensed Chemical Dependency Professional as stipulated in the WAC.

Community Assisted Response & Engagement (CARE) is hiring Community Crisis Responder I's as part of the expansion of the Community Crisis Response (CCR) Unit. These positions will be responsible for being the first-line responder as a part of a team with other CCR team members and will report to a Community Crisis Responder Supervisor.
CARE manages Seattle's 9-1-1 emergency and non-emergency response, working with other Public Safety Departments, including Seattle Police and Seattle Fire, to ensure a safe and healthy community. CARE expands the city's ability to enhance public safety through Dual Dispatch, which provides additional mental health professionals to respond to people experiencing non-violent mental health crises or quality of life concerns.
Seattle 9-1-1 processes more than 900,000 telephone calls for service annually, the majority of which result in the dispatch of police, fire, rescue, and Community Crisis Responders.
The CCR Units operate on shifts to ensure citywide coverage seven days a week. Community Crisis Responders will be scheduled for 40 hours a week with the potential for overtime.
* Provide rapid response and crisis intervention to individuals who have come to the attention of Seattle 9-1-1.
* Assess individuals to rapidly identify urgent needs related to substance use disorder, mental illness, environmental threats, and personal safety to determine best course for care or to recommend ongoing case management (medical care, substance use treatment, therapeutic support, etc.)

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. The Adoption Social Worker position requires travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.

The DESC PACT team is centered in the concept that recovery is more successful when all providers work closely together to provide integrated support services for all aspects of participant's lives, including medication, therapy, social support, employment and housing.
Team scheduling requirements reflect this commitment to provide participants with intensive wrap-around, 24/7 services at their homes and in a variety of community settings. Team members provide rotating coverage for 12 hours per day Monday to Friday and 8 hours per day on weekends & holidays. In addition, all team members participate in on-call rotations to provide 24-hour crisis coverage.
MAJOR JOB RESPONSIBILITIES:
Manage and facilitate PACT's SUD Programming:
The PACT model requires the SUDP to divide their time into approximately 80% facilitation of the SUD program and 20% direct case management work.
The PACT SUDP will update substance use treatment plans monthly according to the client's identified stage of change and propose effective, evidence-based interventions to meet each stage.
Act as the PACT team's subject matter expert for substance use disorders.
Provide consultation to other team members regarding SUD issues, treatment, and education, including Harm Reduction best practices.
Provide staff training on substance use issues, as appropriate.
Provide individual and group substance use disorder (SUD) treatment in office and in community settings using a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance use, and has client-determined goals. With the client's participation, develop support and treatment plans; facilitate linkages to collaborative resources when appropriate.
Plan, organize and facilitate treatment and support groups for clients, as appropriate.
Monitor and coordinate replacement of PACT harm reduction supplies.
Advocate for client access to community resources and services, ensuring that client needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.
Develop and maintain strong collaborative relationships with DESC staff, and other service and resource organizations to ensure full continuity of care for clients.
Participate in psychiatric consultation, supervision, program team meetings and in-service trainings; participate in clinical reviews and case conferences.
Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned.
Comply with data gathering/submission requirements, and with applicable program research and evaluation procedures.

Participate in the planning, organizing and facilitating of unit mitigations for clients on your caseload. POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.

Team members provide rotating coverage for 12 hours per day Monday to Friday and 8 hours per day on weekends & holidays. MAJOR JOB RESPONSIBILITIES:
* Manage and facilitate PACT's SUD Programming:
* The PACT model requires the SUDP to divide their time into approximately 80% facilitation of the SUD program and 20% direct case management work.
* The PACT SUDP will update substance use treatment plans monthly according to the client's identified stage of change and propose effective, evidence-based interventions to meet each stage.
