The Middle School Math Teacher will be responsible for the successful completion of the following tasks: * Represent a commitment to equitable practice in all areas within the job description; this includes use of evidence to make decisions, holding high expectations for all students and committing to providing individualized support to all students to meet those expectations; * Manage a homeroom of students; * Support the instructional program with asynchronous recorded lessons and synchronous instruction, along with individual communications and intervention; * Provide clear, constructive, and consistent feedback for all student work, create progress reports and conduct parent conferences in a timely manner; * Communicate with parents, students and other teachers on a regular basis to develop and update Personal Learning Plans and schedules, score assessments, provide feedback on student work, suggest instructional approaches and strategies, monitor completion of assignments and coach special projects; * Develop a general knowledge of the entire program's 3-12 curriculum and a very detailed knowledge of the courses for which responsible; * Operate within the Response to Intervention/Multi-Tiered System of Supports model, using data and evidence to provide Tier 1 differentiated instruction to all students while working with a team of teachers to develop and implement Tier 2 and 3 interventions; * Support students and parents with alternate lessons, assessments, and strategies and provide additional assistance with daily assignments and projects in order to meet individual learning goals; * Build strong relationships with students and families to provide direct instruction and intervention that meets the needs of the individual student; * Work collaboratively with other staff within a professional learning community to ensure that all students are successfully progressing through the program, that parents have a central point of contact, and that tasks are distributed among staff; * Collaborate within the school's professional learning community to continuously monitor student data to make strong instructional decisions that have direct impact on student learning; * Collaborate with other teachers, team members and staff learning specialists to develop alternate interventions and extensions to students' programs to ensure all students learn at relatively high levels of achievement; * Keep student records and data up-to-date, including cumulative files, online student and family information, attendance accounting, and logging all student and parent contacts; * Work with Advisory Teachers and School Counselor(s) to ensure students and families are receiving appropriate communications, students are making adequate progress and established goals are being met; * Work with other teachers to coordinate and attend social activities, required school events (Graduation, for example), relevant field trips for students, making consideration for integrating these trips into the curriculum; * Actively engage with students and families at school events and field trips; * Devise and implement virtual methods of creating and maintaining a "school community"; * Collaborate in a Professional Learning Community (PLC); * Attend required school and district professional development, in addition to seeking professional growth within curriculum field(s) of study; * Participate in the organization and administration of State Testing, as directed; * Proctor state testing, under the supervision of the Campus Test Coordinator, the Field Administrator and the Test Site Director at a physical location; * Participate in student recruiting sessions and other marketing efforts that require teacher representation; * Other duties as assigned. Pharmacy Administration provides leadership and direction to the various pharmacy departments and programs at SHS. These

operations. We are staffed with seasoned pharmacists and technicians who help optimize pharmaceutical care throughout our SHS

EXPERIENCE/EDUCATION/QUALIFICATIONS

Current unencumbered Certified Oregon Pharmacy Technician License required.
Three (3) years Certified Pharmacy Technician experience required.
Experience and/or training with MS Office and database systems required.

The Rehabilitation teams at Samaritan evaluate and treat patients with injuries or conditions that have caused either a decline in physical mobility or functional independence. Our acute services are centered out of Samaritan's Regional Medical system, the 160 bed, level 2 trauma center. Our hub status affords us a complex patient census that is among the top 5 in acuity in the state. Our newly built outpatient clinic features cutting edge equipment, separate adult and pediatric gyms, and an underwater treadmill. Additionally, we offer outpatient services from the newly constructed Pastega Regional Cancer Center.

The Diagnostic Imaging teams at Samaritan Health Services use state-of-the-art imaging technology while providing health care teams with the information needed for a fast, accurate diagnosis. EXPERIENCE/EDUCATION/QUALIFICATIONS
Current ARRT RT(R) credential required. If registry eligible, credential within six (6) months of hire required.
Current unencumbered Oregon license in Radiologic Technology required. If a new graduate, temporary license upon hire and permanent license within six (6) months of hire required.
Healthcare Provider BLS required.

As an Adaptive Physical Educator you will accelerate student outcomes in Grades K-12 by delivering designated physical education services aligned to each learner's individualized education plan (IEP) - on time, on schedule, and to standard. This role elevates instruction through evidence-based curriculum and community-based learning consistent with IEP intent; builds individualized schedules that advance service goals; and documents progress with rigorous, data-driven reporting.
‖ Responsibilities Include:
Implementing designated services as detailed in each student's IEP/ITP, meeting required weekly minutes within the specified service windows.
Developing and implementing standards-aligned, evidence-based curriculum consistent with the letter and intent of the IEP/ITP, including on-site and community-based instruction as monitored by the supervisor.
Developing and implementing individualized schedules that directly address each student's IEP/ITP goals related to designated services.
Consulting with the supervisor regarding student progress and the implementation of the IEP/ITP/PBIP, and supporting transition planning for mainstreaming/inclusion or post-secondary/adult program pathways.
Conducting ongoing evaluations of student needs; compiling IEPs/ITPs and Progress Reports; and reporting data to the supervisor, parent/caregiver, and district representative.
Recording and maintaining accurate, timely data within student programs, classroom records, and assigned data systems.
Assisting teachers and classroom staff in implementing related-service curriculum by modeling best practices and preparing materials.
Participating in IEP meetings as requested; presenting proposed goals and current levels using data-based evidence; and fielding goal-related questions.
Providing direct services through collaborative consultation with instructional and clinical teams.
Preparing individualized materials and adaptive equipment to maximize student access and engagement.
Facilitating student transitions to the least restrictive environment through coordinated planning and skill generalization.
Completing all required basic competency training and courses, including Supervision Training and other compliance modules.
Completing all staff development necessary to maintain the credentials required for this position.
Observing and training staff to implement related-service goals consistently across the school day.
Assisting and mentoring classroom staff at the supervisor's request while meeting all ongoing credentialing and professional development requirements.
Attending required meetings, including supervision sessions, all-staff meetings, annual planning, and action-team forums.
Adhering to organizational communication guidelines by using objective, professional language (NORMS) with colleagues and all stakeholders.
Giving and receiving feedback professionally-prioritizing appropriate timing, setting, confidentiality, and data-based rationale.

‖ Qualifications Required:
Bachelor's degree or higher in education or a closely related field of study.
Licensed currently or in the process of obtaining an adaptive physical educator (APE) credential.
Licensed currently or in the process of obtaining a special education credential preferred.
Ability to obtain and maintain certification in company approved crisis management (PCM) training.
Prior experience working with adaptive physical education services, preferably in an alternative, special education and/or behavioral health program setting.
Prior experience and/or knowledge in special education services and compliance, particularly for students with autism related disabilities, emotional disturbance and/or behavioral disorders.
Prior experience and/or highly knowledgeable in working with students with individualized education plans (IEP's).
Well-versed in applied behavior analysis (ABA), positive behavior interventions and supports (PBIS), effective teaching strategies, classroom management systems, data-based decision making, functional behavior analysis (FBA) and behavior intervention plans (BIP's).
Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
Ability to think and act quickly and calmly in an emergency and make independent decisions.
Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability.
Knowledge of relevant technology including experience with instructional technology, Microsoft Office Suite, database entry and basic office equipment.

The Rehabilitation teams at Samaritan evaluate and treat patients with injuries or conditions that have caused either a decline in self-care, physical mobility or functional independence. We provide specialty care from pediatrics to geriatrics, from neurological to women's health, from lymphedema care to wheelchair fitting, hand therapy and more. Our acute services are centered out of Samaritan's Regional Medical system, a 160 bed, level 2 trauma center. Our outpatient clinic features cutting edge equipment, custom splinting stations, a Sanet Vision Integrator, a therapy pool with an underwater treadmill and separate adult and pediatric gyms. We also offer outpatient services at the Pastega Regional Cancer Center. All outpatient services are one-on-one and feature therapist-driven treatment times.

The Diagnostic Imaging teams at Samaritan Health Services use state-of-the-art imaging technology while providing health care teams with the information needed for a fast, accurate diagnosis. Imaging services include: X-Ray, Fluoroscopy, Bone Density, CT Scan, Digital Mammography, MRI, Nuclear Medicine, PET-CT, and Ultrasound services.
EXPERIENCE/EDUCATION/QUALIFICATIONS
Current ARRT RT(R) credential required. If registry eligible, credential within six (6) months of hire required.
Current unencumbered Oregon license in Radiologic Technology required. If a new graduate, temporary license upon hire and permanent license within six (6) months of hire required.

Texas Connections Academy @ Houston (TCAH) is a Texas Online School that offers 100% virtual instruction to public school students across Texas in grades 3-12 who are not physically present on campus during instruction. The TxVSN OLS program is authorized by Texas Education Code (TEC) Chapter 30A and administered through TEA. Connections Education, LLC is in partnership with Houston ISD to provide this high quality and rigorous virtual instruction through TCAH.

Position Summary and Responsibilities
Working from your home office in Texas, the Advisory Teacher will virtually assist students and parents/learning coaches with course selection, scheduling and will be the initial point of contact for student concerns that span multiple subject areas as well as non-academic issues. The Advisory Teacher will become an expert on course and credit requirements and will work with the School Counselor to establish counseling processes for middle and high school students.
The High School Advisory Teacher will be responsible for the successful completion of the following tasks:
Advise students and families related to academics;
Keep current on all high school graduation requirements, including special requirements such as community service, and communicate this information to the High School Coordinator and to the principal;
Assist students and parents with the secondary course selection process according to specific state credit requirements and student needs and interests;
Advise students with issues related to dropping courses and changing schedules, seeking support as needed, while ensuring that the school's course selection and drop policies are adhered to;
Review student transcripts and the entry of credits into the online transcript system;
Follow procedures to ensure that Connections Academy transcripts are accurate and up-to-date;
Work with the administrative assistants to obtain school records from the student's previous school (where applicable) and forwarding student records when appropriate;
Implement programs for students and families related to interpersonal adjustment issues;
Work with school teams to identify and help families access school and community resources;
Participate in the development of crisis prevention and management plans for the school and serve as a key member of the Student Support Team (SST);
Coordinate a team of middle and high school teachers to help identify students who are “at-risk” or “in crisis”; Act as the main point of contact for these students and families, communicating regularly with them;
Oversee and maintain the Personal Learning Plan (PLP) for each student;
Implement efforts to secure complete and accurate records for Connections Academy students;
Support families through the school withdrawal process, assisting with data collection regarding withdrawal;
Report and refer critical incidents that jeopardize student well-being as obligated by law, administrative regulations, or ethical standards. Stay current on all state regulations relating to incident reporting and documentation;
Implement special programs such as Advanced Placement support, SAT and ACT preparation, and college entrance preparation;
Understand the requirements for and assist the principal to facilitate the administration of all high school testing, including exit exams, PSAT, SAT, ACT, and AP exams;
Help students to research and understand their post-secondary career and educational options;
Assist with student preparation of applications for college and jobs;
Educate students and parents about college admissions requirements and financial aid;
Support the coordination of high school graduation ceremonies;
Support professional development activities for teachers and school staff members;
Assume teaching and grading duties as necessary including teaching middle school elective courses;

Requirements
Texas teacher certification in a secondary content area or grade level; ACP candidates are not eligible
Must hold or be willing to immediately obtain TxVSN Certification
Strong technology skills (especially with Microsoft Office products and Google Suite)
