Job Responsibilities and Duties:
Case Managers at Pinnacle Services have the opportunity to work with persons served on CADI, DD, or AC/EW waivers. These persons served may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.
The Case Manager Is Responsible For:
Assessing the areas of the persons served life where assistance is needed.
Offering and coordinating resources for the persons served being supported.
Monitoring service delivery and working with providers to ensure the individual's needs are met.
Developing service plans.
Attending regular in-person meetings with persons served and their interdisciplinary teams.
Maintaining and Creating accurate Case Notes and Billable Units.
Staying up to date with case management systems, software, and DHS and County resources.
Salary:
Non-LSW wage: $49,000/year salaried for 40hrs/week.
LSW wage: $51,600/year salaried for 40hrs/week.

Position Summary
Work within our adolescent (11-18) day treatment programs, planning and facilitating group skills services to clients of the program. This position works closely with the program therapists and other Life skills counselors to create and maintain an environment that is sensitive to the unique mental health needs of each client, assisting them in learning how to manage their mental health symptoms so they can function at home, in school and within the community. This position floats between 3 of our child day treatment centers located in Forest Lake, Oakdale, and North St. Paul.
Additional job duties include working collaboratively with client's family, treatment team and school, as well as developing and implementing behavioral strategies to intervene and decrease disruptive behaviors. Minimum qualifications: BA/BS in a Human Service-related field, with 1 to 2 years' experience working with severely emotionally disturbed students in an education or treatment center.

Document all services provided in the Electronic Medical Record based on the requirements of Certified Community Behavioral Health Clinics and Minnesota Statute 245G including comprehensive substance use assessments, integrated treatment plans, progress notes, chart notes, treatment coordination and referrals, and discharge summaries.
Meet with the client's family and/or significant others as appropriate. Educate and offer suggestions for how they best can support the client's recovery program.
Facilitate evidence based, trauma informed, and person-centered services in groups and individual sessions.
Attend weekly supervision with the Treatment Director and substance use and recovery staff meetings.
Coordinate and facilitate communication for the client with attorneys, probation officers, mental health providers, etc.
Consistently demonstrate clinical competence and ethical professionalism
Requirements:

Be adequately trained in evidenced based interventions to address and substance use.
Ability to work independently and with teams.
Must be cleared by an acceptable criminal background check based on Spero and Department of Human Services and Department of Education guidelines.

Review detailed reports and information. Use the computer for documentation.
Bilingual preferred.
Use office equipment including telephone, printer, copier, fax machine, etc.

Education and Experience:
Must be a Licensed Alcohol and Drug Counselor (LADC) or Alcohol and Drug Counselor - Trainee (ADC-T) by the Minnesota Board of Behavioral Health and Therapy.

This position may receive a $1.00 to $2.00 per hour add-on pay based on the type of SAC credential possessed at the time of hire.
Pay will be set in accordance with the Compensation Plan and Wisconsin Administrative Code in effect at the time of hire. A probationary period is required.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
To receive the bonus, a candidate must sign an agreement indicating they will remain in that occupational area for a year. If the candidate leaves their occupational area prior to completing the one year, they may be required to reimburse the initial payment.

These positions will normally be on site, but certain sites may have occasional remote work available. You can ask about a specific location if you are offered an interview.
