It will be based upon the needs identified in the initial assessment as well as in the individual's CANS/ANSA assessment. The Comprehensive Care Plan will be reviewed every (90) days. Documentation in the service plan must include the follow:
a. Member name, MCC identification number, and DOB
b. Date of Plan development
c. Documentation of each life functioning domain(s) based upon prioritized needs and desires, as identified in the member assessment. Each domain in which the member is working must contain the following documentation:
1. Measurable long terms goals
2. Measureable Objectives and Interventions.
3. DSM-5 DX.
4. Anticipated Target Date.
5. Documentation of participants in individuals plan.
5. Crisis Plan must be developed within twenty-four (24) hours of enrollment into Mental Health Care Management. This plan is to include the following elements:
a. Member's name and locator information
b. Diagnosis & medical information
c. Plans for and step in crisis resolution process
d. Signs & symptoms of decomposition including predisposing factors
e. Crisis interventions to be avoided.
6. CM will document that it encourages member and family member/caregiver involvement.
7. CM will document that it ensures access to Care Management for members with mental health and substance use diagnosis.
8. CM will provide evidence that it has performance monitoring standards and tracks and monitors outcome date for member receiving CM services.
9. CM will demonstrate cultural competence in the administration, design and deliver of CM services are clinically appropriate
10. CM will obtain and maintain an F endorsement on their driver's license to assist in transportation services as required.
11. CM will provide documentations of at least 1 monthly contact with the service recipient.
12. CM will provide evidence that medical records for each member will contain the following at minimum;
a. Assessments completed prior to the development of the MHCM Comprehensive Care Plan.
b. A MHCM Care Plan developed within thirty (24) hours of enrollment into MHCH, containing the elements described herein.
c. A MHCM Crisis Plan developed within twenty-four (24) hours of enrollment into MHCM containing the elements described herein.
d. Progress notes after each service contact.
e. Current Authorization to Release Forms for member's PCP, Pharmacy, Emergency Contact, Past Providers, and other current Providers.
f. A current Preliminary Discharge Plan updated monthly for ARTF, Quarterly for Enhanced, and every six month for Supportive levels of care and Community Based Outpatients.
g. CANS/ANSA assessment and follow-up.

What will an Assistant Store Manager do?
Assistant Store Manager manages the daily operations of donated merchandise in accordance with established business practices as mandated by the Agency's policies, procedures, and budget projections in an assistant role.
Job functions
Through words and actions support Chattanooga Goodwill's Mission, Vision, and Values; furthermore, actions and decisions will align with organizational guiding principles.
Maintain agency, employee, and participants confidentiality. Violation of confidentiality may be cause for termination.
Punctual and dependable attendance.
Manages budget and maintains expenditures in accordance with the Agency, departmental, and store financial goals.
Hire, train, evaluate, motivate, and supervise all store personnel to maintain and exceed the Agency's operational standards.
Comply with Agency policies and DGR standard operating procedures (SOPs) and the Agency's safety and security policies and procedures.
Greet, assist, and provide excellent customer service to donors and customers.
Assist and resolve any complaints and communicate with the management team.
Maintain proficiency for tasks required in all workstations.
Monitor and maintain overall store appearance and housekeeping to Agency standards.
Perform and assist in all functions completed by store personnel.
Attend all meetings and training as required by the Agency and conduct all monthly meetings with store employees.
Maintain all company records and reports accurately and in a timely fashion.
Meet store sales and production goals by assisting in sales and processing of items.
Review and be familiar with Goodwill Industries of the Greater Chattanooga Area safety policies, programs and procedures and adhere to all safety rules, regulations, and safety codes. Attend safety training and meetings and report any unsafe work conditions, accidents, or injuries immediately.
Assist at other store locations as needed.
Perform other duties and projects as assigned.
Key knowledge and experience
High School diploma or GED, preferred.
Associate degree or completion of related retail management course work preferred.
3 years of experience in a supervisory role of 12 or more employees in retail, manufacturing, production, or food services preferred and/or demonstrated success and completion of Goodwill assigned training.
High level of attention to detail.
Must have excellent oral and written communication skills in English as well as interpersonal skills, bilingual a plus.
Must be proficient in Microsoft Office products such as Word, Excel, PowerPoint, and Outlook.
Must be able to adapt to growth and a changing environment within the Agency.
Ability to complete and maintain: CPR and First Aid certifications.
Ability to speak, read and write the English language.
Reliable transportation and communication required.
Must be available to work during operating business hours as needed for business operations. Work schedule includes days, evenings, holidays and blackout periods, weekends are required.

JOB SUMMARY: This position is responsible for driving company trucks to job sites, transporting material, operating machinery, assisting in additional work as assigned to complete work on the job site. DUTIES AND RESPONSIBILITIES:
Drive both local and regional routes
Drive loads to sites for proper disposal
Participate in practical and manual activities on a daily basis.
Engage in strenuous physical labor including lifting, pulling and pushing heavy objects.
Operate light and heavy equipment including, but not limited to: blasting equipment, vacuum trucks and hoses, etc.
May require work in confined spaces, elevated spaces, hot, wet, cold, and foul smelling environments throughout an 8-12 hour shift while holding various sizes of vacuum hoses.
Work in environments with hazardous materials and excessive noise levels.
Extended work hours, including evenings, and weekends are required. Travel to various locations and working for extended periods of time out of town is as needed is mandatory to support the needs of the business.
Due to the emergency response and operational nature of the position, a continuous condition of employment. All associates in this position must for this position is acknowledge and agree to be on-call 24 hours a day, 7 days a week, 365 days a year and on occasionally will be require to work weekends and holidays.
Complete assigned work on job sites.
Additional duties as assigned.
EXPERIENCE AND SKILL REQUIREMENTS:
Class A CDL license required.
Must be able to pass DOT Medical and Pulmonary Fit Tests.
Must consistently demonstrate safe working practices
Must maintain a valid commercial driver license and a clean driving record.
Ability to read and write in English Required.
Tanker and Hazardous Material license endorsements preferred.
Ability to work occasional weekends and holidays as needed.
Ability to lift up to 75 pounds and perform strenuous physical labor in various work environments.
