GENERAL DUTIES/RESPONSIBILITIES
The following duties and responsibilities include but are not limited to:
1. Treat individuals with respect, dignity and compassion at all times
2. Daily living
* Clean, maintain and participate in the upkeep of the home and properties to ensure client and staff safety
* Assist individuals with:
* Plan, purchase and prepare meals according to individual's dietary needs
* Plan and participate in leisure time and recreational activities
* New social opportunities/experiences to the individuals
* Cope effectively with social, emotional and physical situations

3. Documentation / Communication:
Documentation provided by staff is considered a legal document. Ensure all individual data is recorded accurately in individual's record in Therap and all program documentation is competed on time
* Accumulate and record data relevant to behavioral programs, review individuals progress, developmental needs and barriers impeding progress
* Identify and prioritize individuals' needs
* Complete comprehensive documentation
* Report seizures, behavioral episodes, injury or illness, client concerns, and/or other medically or emotionally related problems to the house manager, supervisor, nurse and team members. Reports suspected allegation of abuse/neglect to the Abuse Investigation Division in a timely manner immediately.
* Report any incidents or evidence of violation of residents' rights to Administrator and/or appropriate parties immediately.
* Ensure regular, professional communication with all team members
4. Trainings
* Attend and successfully complete all mandatory trainings/meetings required by DDS and the agency.
* Successful completion of DDS Medication Administration Certification. Yearly maintenance of Med Certification
5. HIPAA
* Maintain and safeguard individuals' private information. This includes but is not limited to diagnoses, health record, and program documentation.
6. Additional duties as assigned
QUALIFICATIONS:
* Bachelor's degree preferred or High school Diploma with 2 years' experience working with individuals with developmental disabilities.
* Must have Medication Administration Certification or must be able to obtain Medication Administration Certification upon the next available Med Cert Course immediately following hire.
* Demonstrates organizational ability, communicates effectively with individuals served and members of the team.
* Applicants will be subject to criminal and driving history background checks. Must have a valid Connecticut driver's license with no more than 2 violations in a three year period, subject to review. Drivers' History completed annually.

Description:
Our DirectSupportProfessionals are committed to facilitate, model, and build skills that will lead to the optimal independence of the people we support.
Essential Functions:
Deliver training, support, and assistance to participants in the areas of personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
Participate in the design, implementation and monitoring of the participants individualized support plan. Your observations and opinions are crucial to tailoring an achievable plan.
Accurately administer prescribed medications to participants as ordered by physicians.
Accurately complete required documentation including but not limited to program data, incident reports, daily log, progress notes, and health records.
Always ensure participants health and safety including but not limited to prevention of abuse, perform CPR, perform Standard First Aid, perform Heimlich procedure, and evacuate participants in case of a fire.
Providing transportation to recreational and community activities
Prepare participants meals following published menus.
Must have the ability to mentor and assist people in all areas of life skills including recreation, independence, communication, and health

Minimum Qualifications:

Valid driver's license required.

III. Program Management
Assist with intakes and the discharge of participants in the program, which includes data entry of required information for tracking purposes.
Provide crisis intervention counseling as necessary.
Transportation of clients, as required.
Prepare monthly report of caseload for DCF as per assigned deadline, which includes summary of each individual relative to life skills, money management, interpersonal skills, etc.
Administration of medication; maintenance of appropriate documentation.
General maintenance of daily/shift documentation.
Ensure program compliance at all times.
Participate in weekly team meetings to review each participant and determines appropriate courses of action.
Provide adequate supervision at all times (minimum ratio 1:6), to ensure resident support and safety.
Demonstrate positive rapport with residents, including establishing professional boundaries.
Exercise effective group skills and techniques.

Qualifications: Bachelor's Degree in Human Services or related field highly preferred. Experience working with adolescents as well as case management experience required. Review and prepare shift change logs at the beginning and end of each shift, as well as orally summarize pertinent information for aides coming on duty
Inform a licensed nurse of any changes in the resident's condition or needs
Answer emergency call system and respond appropriately
Report all incidents to a licensed nurse and in-coming staff
Follow proper procedures for the Self-Administered Medication Management system
Assess capabilities to determine needs for additional assistance
Assist with housekeeping duties, including resident laundry according to schedule or as needed.
Perform dining room duties and assist with the serving of residents
Assist relatives, guests, and visitors, as needed
Assist with activities programs
Maintain a positive, professional calm, image, and environment
Take a team approach to performing duties
Complete all required annual in-service training
Adhere to all OSHA regulations and safety procedures when bending, lifting, and using equipment
Requirements:
Must be currently certified as a nursing assistant or have successfully completed the certified home health aide program.
Must understand the physical and emotional needs of elders, enjoy working with them, show understanding and patience and perform duties required to enhance the resident's environment and quality of life.
