The RN Hospice Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.
Our Hospice Nurses work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care. They provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.
Hospice Nurses supervise the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conduct supervisory visits per regulatory guidelines.
Hospice Nurses provide education, supervision and counseling to the client and their family regarding home nursing procedures and other care needs as appropriate to client's needs. They assure that the client and their family are involved in the development of the plan of care whenever possible.

* Report seizures, behavioral episodes, injury or illness, client concerns, and/or other medically or emotionally related problems to the house manager, supervisor, nurse and team members. Reports suspected allegation of abuse/neglect to the Abuse Investigation Division in a timely manner immediately.
* Report any incidents or evidence of violation of residents' rights to Administrator and/or appropriate parties immediately.

4. Trainings
* Attend and successfully complete all mandatory trainings/meetings required by DDS and the agency.

PRIMARY RESPONSIBILITIES:
Attend and actively participate in pre-camp orientation and staff meetings.
Live in a cabin with assigned group and assist in Cabin Life
Eat meals and attend evening activities with assigned group.
Accompany campers to scheduled programs and actively support them through all stages of the activity.
Plan and initiate meaningful activities and events during quality time/free periods.
Work with Group Leader to develop cabin skills programs, quality time activities and camper hobbies.
Work cooperatively with other cabin staff to support campers.
Facilitate building camper self-esteem through Positive Practices: campers experience success, they have choices, they engage in a variety of activities, they have opportunities to be independent, and there is a natural flow of meaningful activities.
Demonstrates timely arrival to assigned location and throughout workday and follows the professional expectations as stated in Horizons' Policies and Procedures.
Exhibits positive, effective management and observational skills. Implements positive practices under the guidance of Horizons' Mission and Guiding Principles.
Communicates in a way that is accurate, timely, positive and capacity-based, focus-person centered and respectful of focus-person and stakeholder's privacy.
Follows all policies and procedures of Horizons (including mandated reporting).
UConn Regional Campus Nurse Navigators (RN) - Stamford Campus
Part time job in Storrs, CT
Under the general supervision of the Regional Nurse Coordinator, this position provides professional nursing and health services to the UConn Stamford campus. As a college health nurse, this registered nurse will be responsible for providing patient focused advice and education, by supporting the maintenance and improvement of the health of students, identifying, and assisting in removing or modifying health related barriers, and supporting early detection and early intervention strategies. Collaborates with other providers to enable the student to attain and maintain health. Refers students to other internal or community-based resources as appropriate. Maintains electronic medical records and ensures compliance with state laws and regulations, as well as SHaW policies and procedures, and program protocols. May include nursing assessments, examinations, and treatment on the Storrs campus.

DUTIES AND RESPONSIBILITIES
* Provides in person and telephone advice and consultation with regard to treatment and prevention.
* Assists in the coordination of care and access to health resources within the community.
* Collaborates with healthcare team to formulate individualized plan of care for student patients.
* Provides specific health counseling and general health education to individuals, patients, and groups.
* Refers and assists patients in securing appointments for specialized medical care beyond the scope of the Student Health and Wellness services, which may include supporting students in securing transportation and insurance solutions.
* Participates in and/or conducts in-service presentations. Participates in various SHaW and university committees and advisory groups.
* Interviews patients, documents medical history, may examine patients, and make assessments and/or physical nursing diagnosis of condition of such patient. Counsels' patients regarding treatment and prevention.
* Administers medications and treatments, provides care via established protocols as authorized by licensed Physicians and Nurse Practitioners.
* Maintains proper electronic clinical records and reports. Maintains drugs, medical supplies, and equipment, as necessary.
* Performs related duties as assigned.
MINIMUM QUALIFICATIONS
* Licensed as a Registered Nurse in the State of CT
* Bachelor's degree in nursing or other health related field
* Two years of nursing experience working directly with adolescents/young adults in community health, ambulatory care, or school-based nursing environment.
* Demonstrable physical assessment nursing skills
* CPR Certification
* Must have a demonstrated understanding of and stated commitment to diversity, inclusion, and equity issues.
* Demonstrated experience working with diverse populations.
PREFERRED QUALIFICATIONS
* Five or more years of Nursing experience
* Experience working in a College Health Setting.
* Experience providing Nursing Telephone Triage/Advice.
* Experience in a Case Management /Nurse Navigator role.
* Bilingual experience (Spanish preferred).
* Previous experience with emergency medicine.
APPOINTMENT TERMS
This is a permanent part-time in-person 12-month, 21 hours a week position. This opening is for a primarily day position with a flex schedule which may include an occasional weekend day. Candidate must be willing to extend into the evening hours on occasion to meet the needs of the service/population. Salary will be commensurate with qualifications and experience. A requirement to be present on the Storrs campus for a period for orientation may be required.
