Job Summary and Qualifications
As a Case Management Assistant you will be joining a collaborative team that is dedicated to serving others in need and also supporting one another. In this role you will:
* Coordinate with RN case manager and social worker for support needed. Communicate updates on progress throughout the day. Refer patient questions back to the case manager/social worker.
* Provide clerical support in multidisciplinary rounds, including updating NATE Tempo at direction of team.
* Make referrals through the web-based referral system (nH Discharge/other) to the specific post-acute care (PAC) providers requested. Includes sending the appropriate packet of information from the medical record required for the level of care.
* Monitor patients with PAC referrals and intervene to expedite discharge.
* Prioritize workload to ensure a timely contact and intervention with PAC providers.
* Send additional information to PAC provider when requested.
* Communicate with RN case managers/social workers regarding the status of PAC referrals, bed availability, and other barriers that cannot be resolved.
* Communicate results of PAC referrals with the patient, including having the Patient Choice letter signed.
* Monitor Medicare patient's planned discharge dates and deliver the Important Medicare Message (IMM) to the patient per facility policy.
* Deliver the Beneficiary Notice to patients identified as being in the BPCI-A program (Bundle Payment hospitals only).
* Obtain physician signature on required Case Management documents and forms.
* Based on facility practice, set up transportation, send information to Transfer Center, and generate discharge packets for PAC transfers.
What qualifications you will need:
* Associate Degree preferred
* Medical knowledge and terminology required
* Minimum one year of experience in a health care setting required
* Health care professional (C.N.A., E.M.T.) preferred
* Ability to establish and maintain collaborative and effective working relationships.
* Ability to communicate effectively in oral, written and electronic formats.

As a Microbiology Technologist at our facility you will be an integral part of our Laboratory team. * You will be responsible for receiving blood specimens, performing blood tests, and studying the morphology of the components of blood.
* You will perform various routine or special clinical laboratory tests in several sections of the Laboratory using automated equipment, manual testing, and microscopic analysis to obtain data for use in diagnosis and treatment of disease.
* You will be responsible for maintaining and using various equipment in the Laboratory including automated, chemistry analyzers, ion-specific electrodes, osmometers, and other analyzers.
* You will perform antibody screening, antibody identification, and cross-matching procedures required in the administration of blood/blood components and identify antibodies in donors or patient's blood that could react adversely during transfusion.
* You will notify nursing units and/or physicians of critical lab values.
* You will implement new testing methods, products, and instrumentation as appropriate to keep department current with technology.
Qualifications:
* Bachelor's degree in Medical Technology (MT), Medical Laboratory Scientist (MLS), or related sciences including a practical training program.
* Associate's degree is acceptable with appropriate years of experience.
* Nationally recognized certification as a Medical Technologist (MT/MLS or MLT) from the American Society for Clinical Pathologists (ASCP) or equivalent.
* 3-5 years of laboratory experience.
