Responsibilities
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes
Review patient care plans daily to determine if changes in the resident's daily care routine have been made on the care plan
Inform the nurse supervisor of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan
Assist resident's with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments
Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment
Ensure residents' rooms are ready for receiving and help residents feel comfortable
Participate in facility surveys by authorized government agencies
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines
Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator
Qualifications
Have a 10th grade education or above
Be a licensed certified nursing assistant in this state
Must be able to read, write and speak English fluently.

We are in search of experienced, dynamic advanced practice providers to join our Outreach team. As a primary care medical home, we serve 28,000 patients annually throughout our seven health care centers, dental clinic, and mobile health center. We are an innovative non-profit organization, with the focus on providing exceptional health care to all who have barriers to care and include integrative behavioral and dental health care in our practices.
At all our primary care sites, our multi-disciplinary teams focus on providing excellent medical care and supporting our patients' emotional and social needs, both adults and children. These teams include a mix of physicians, family nurse practitioners, physician associates, integrated behavioral health team, nurses, MAs, registration personnel, and team leads. Each team has a clinical leader, and our practice also employs a psychiatric nurse practitioner and a child and adolescent psychiatrist. This role will work on our Mobile Health Center (various sites) and at the OHRA Center (Ashland, OR), which focuses on serving our most vulnerable community members and reaching those who are unable to gain access to our health centers. About the Position
We are looking for an On Call Residential Care Associate I to join our team in the Southern Oregon region! The On Call Residential Care Associate I will provide caring companionship and assistance with daily tasks to clients. They will provide personal care services and support that helps individuals develop appropriate daily living skills and independent activities of daily living to increase or maintain their level of functioning. The On Call Residential Care Associate I will assist with meal preparation, clean-up, and food purchasing, while encouraging client participation. Under the guidance of a Registered Nurse, they will pass medications and perform delegated medical care as prescribed by the client's primary care provider. They will transport clients to and from the program as necessary to engage and provide treatment services. They will maintain compliance with established productivity standards for documentation of services.
Please keep the following things in mind:
On Call staff must work a minimum of 32 shift hours per month
On Call staff must attend a minimum of one staff meeting per month (and clinical supervision if a QMHA)
On Call staff must stay current on company training
On Call staff must respond to facility requesting coverage within 2 hours of initial call. In some cases, more notice can be provided.
This position falls under the AFSCME bargaining unit. Wages, benefits, and working conditions are set in accordance with the collective bargaining agreement.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.

Training Program

Position Summary
Under the direction of the Health Center Manager, the Health Care Assistant (HCA) will work as a member of a team that operates from established procedures to ensure compliance and efficiency in the day-to-day operations of the reproductive health center and facilitate high quality, inclusive patient care.

Essential Duties

Calling patients when necessary and answering patient questions.
Obtain and prepare lab samples in accordance with established protocol, file laboratory reports, maintain the lab area and lab equipment, and document quality control measures.
Perform routine clinic maintenance to ensure a safe and sanitary environment.
Adhere to affiliate goals and policies on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints.
Attend special meetings and assist Clinic Manager with new hire training as necessary.
Stay up to date with the policies and procedures of Planned Parenthood to ensure compliance with pertinent government regulations including OSHA, CLIA and HIPAA.

Responsibilities
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes
Review patient care plans daily to determine if changes in the resident's daily care routine have been made on the care plan
Inform the nurse supervisor of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan
Assist resident's with daily needs, including preparation for activity and social programs, and transporting residents to/from appointments
Create and uphold an atmosphere of warmth, patience, enthusiasm, and foster a calm and cheerful environment
Ensure residents' rooms are ready for receiving and help residents feel comfortable
Participate in facility surveys by authorized government agencies
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines
Effectively communicate necessary resident information to charge nurses, director of nursing, and/or administrator
Qualifications
Have a 10th grade education or above
Be a licensed certified nursing assistant in this state
Have a passion for caregiving and enhancing the quality of life for our residents
Must be able to read, write and speak English fluently.
Must have an active CPR/BLS Certification.
