Shift: Full-Time, 8:00 AM - 4:00 PM, including every other weekend
Key Responsibilities:
* Implement and monitor patient activities in the Care Connections program.
* Assist with functional mobility, range of motion (ROM) exercises, and application of orthotics as recommended.
* Maintain accurate records of patient participation and communicate daily with the therapy team.
* Report any changes in patient condition to the appropriate staff.
* Support patient rights, privacy, and dignity while adhering to HIPAA standards and facility policies.

Knowledgeable about each household resident's ISP.
Provide meal preparation under the direction of the Food Service Manager
Clean resident room and common areas. This includes dusting, vacuuming, floor cleaning, cleaning bathrooms, keep kitchen tidy and clean.
Provides recreational activities to residents, according to residents' preference and under the supervision of the Case Manager.
Work well with household and neighborhood teams to maximize resident potential, comfort, and happiness.
Perform residents' personal laundry.
Encourage resident involvement in household life, encouraging them to participate in household tasks to the extent their condition permits and the resident is interested.
Follow procedures to provide resident care: bathing, oral hygiene, transferring to/from bathroom, use of commode, keep incontinent residents clean and dry at all times, change bed linens as necessary.
Transfer, ambulate and position residents to ensure comfort, prevent injury and to promote optimal levels of function.
See that residents are properly clothed at all times; i.e. clothing not soiled.
Follow standard procedures to provide personal grooming: trims nails, combs hair, shaves male residents daily and shampoos hair as indicated and informs supervisor if changes occur.
Make beds, keep night stands, dressers and closets in orderly fashion.
Assist resident at meals, serves, distributes beverages as needs, assist residents with cutting foods as needed, provides nourishment when indicated, encourages and monitors meal acceptance.
Prepare residents for tests, X-rays, visits from physicians/dentists/therapists as directed by Supervisor.
Perform personal services for residents such as writing/reading letters, using TV and radio. Promotes comfort, security, independence, and resident safety.

Maintain appropriate techniques in all resident care to prevent spread of disease and infection.
