Position Summary
The Licensed Practical Nurse is responsible for delivering age appropriate, population-specific patient care while embracing HHHN's core values of Quality, Appreciation, Creativity and Sustainability to help support our Mission and Vision.
While administering care to patients under the direction of the responsible RN or medical provider, the LPN must be able to perform each essential duty satisfactorily in order to be successful:
Be a trusted resource for nursing best practices, policies and procedures.
Collaborate with other staff members to develop strategies that continuously improve quality of care and the patient experience.
Provide direct and indirect care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
Protect patients and employees by following infection control guidelines.
Prepare patients for their appointments by following set procedures.
Demonstrate proper medication and vaccine management.
Review and update the patient's medication list and prepare prescription renewals.
Demonstrate complete, accurate, and timely documentation in Athena, our electronic medical record system.
Answer patient phone calls and engage medical provider or RN appropriately
Reinforce patient and family education plans that meet the needs of the patient
Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
Help keep patient visits running smoothly and work with the supervisor to troubleshoot any issues
Mentor and train new and current staff members based on skill set
Participate in team meetings and serve on committees to support clinical quality initiatives
Help manage and track medical supplies, medications, and equipment deliveries
Complete routine health center tasks as assigned (daily, weekly, or monthly).
Perform other duties as requested.
Qualifications
Unrestricted current LPN license to practice nursing in New York State.
Must possess a certificate from an accredited school of nursing.
Minimum of one-year clinical experience (preferred).
Current Basic Life Support certification required.
Ability to travel to other Health Centers throughout the Network, as needed.
Proficiency with Microsoft applications and electronic medical records.
Excellent verbal and written communication skills.
**As a FQHC, Hudson Headwaters requires all new hires to provide original copies of any license, parchment, GED, and/or High School Diploma equivalent, if applicable, during the onboarding process**

Position Summary
The Licensed Practical Nurse is responsible for delivering age appropriate, population-specific patient care while embracing HHHN's core values of Quality, Appreciation, Creativity and Sustainability to help support our Mission and Vision.
While administering care to patients under the direction of the responsible RN or medical provider, the LPN must be able to perform each essential duty satisfactorily in order to be successful:
Be a trusted resource for nursing best practices, policies and procedures.
Collaborate with other staff members to develop strategies that continuously improve quality of care and the patient experience.
Provide direct and indirect care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based.
Protect patients and employees by following infection control guidelines.
Prepare patients for their appointments by following set procedures.
Demonstrate proper medication and vaccine management.
Review and update the patient's medication list and prepare prescription renewals.
Demonstrate complete, accurate, and timely documentation in Athena, our electronic medical record system.
Answer patient phone calls and engage medical provider or RN appropriately
Reinforce patient and family education plans that meet the needs of the patient
Perform ordered point of care testing (CLIA), including specimen collection, and proper documentation.
Help keep patient visits running smoothly and work with the supervisor to troubleshoot any issues
Mentor and train new and current staff members based on skill set
Participate in team meetings and serve on committees to support clinical quality initiatives
Help manage and track medical supplies, medications, and equipment deliveries
Complete routine health center tasks as assigned (daily, weekly, or monthly).
Perform other duties as requested.
Qualifications
Unrestricted current LPN license to practice nursing in New York State.
Must possess a certificate from an accredited school of nursing.
Minimum of one-year clinical experience (preferred).
Current Basic Life Support certification required.
Ability to travel to other Health Centers throughout the Network, as needed.
Proficiency with Microsoft applications and electronic medical records.
Excellent verbal and written communication skills.
