How you will change lives
As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.
Other Requirements
Must meet any practice requirement(s) for the applicable state.
Additional license requirements may be applicable depending upon state.

Summary Description: The Residence Counselor is responsible for tasks relating to the daily operation of the licensed residential services. The Residence Counselor will serve as a role model, providing a home-like atmosphere in which dignity and respect enhance each person's existence. They will participate in person-centered service planning and provide restorative services for each resident. This means keeping things organized around the house and making sure we know where everyone is so that everyone feels safe and at home! The residents we support are independent and thrive when staff embrace creativity and fun.
Responsibilities:
Counsel and assist residents in the development of necessary skills required in decision-making, problem solving, interpersonal skills, and goal acquisition, and document their progress on a regular basis.
Supervise and assist residents in daily living activities, including personal health care and grooming, individual clothing care, personal belongings, bedroom and living quarters.
Supervise and teach (through demonstration) food and meal preparation, purchase and storage, including sound nutritional practice; use of household and small appliances; and use of public transportation.

- JD from an established and reputable law school with 3-5 years of experience

Who We Need
We are looking for patient, energetic and passionate Behavior Support Specialist for our LaGrange School program located in Poughkeepsie, NY. This is an opportunity to work with an interdisciplinary team, focusing on the educational programming for all school-age children at every level of ability. This position will provide behavioral support for students in the classroom to increase student independence through age appropriate, functional means.
Schedule: Monday- Friday 8:00am-3:00pm, 32.5 hours per week, 48 weeks per year (which includes a 6-week summer program)
Pay Range: $17.82 - $19.82 per hour
Responsibilities:
Collect data as part of a functional behavior analysis.
Monitor behavior and strategies and interventions accordingly
Collaborate with classroom teacher on appropriate use of positive reinforcements.
Manage challenging behaviors of students in classroom using research-based practices.
Follows the classroom schedule ensuring that individual student's IEP, specifically behavioral goals are achieved.
Assists in the classroom to conduct individual and group activities.
Attend and participate in staff meetings and job skill training.
Attend therapy sessions with students to assist with behavior management and to allow for appropriate carryover in the classroom.
Assists in the organization and arrangement of the classroom as part of student's therapeutic behavior plan.
Escorts and assists students (via wheelchair, walker, etc.) to and from school buses while providing opportunities for students' increased independence in mobility and orientation.
Attends all specials with students (PE, art, music, etc.)
Takes off and puts on outdoor clothing as necessary and provide opportunities for students' increased independence in self-dressing and undressing.
Toilet children and use appropriate adaptive equipment if necessary. This should be done at least 3 times daily (more often when necessary.)
Change clothing when wet or soiled.
Change diapers and/or sanitary napkins as necessary.
Set up computers, communication device systems and related adaptive equipment.
Adhere to agency Code of Legal and Ethical Behavior, which require all employees, consultants, board members, volunteers and affiliates to perform their responsibilities according to ethical and legal standards with honesty, integrity, fairness, good faith and respect for others and the law.
Follows all agency policy and procedures and demonstrates continuous regard to personal safety and the safety of others.
Performs other appropriate job-related activities as requested by your supervisor or as circumstances warrant.

Requirements
