Perform a two-nurse narcotic count at the beginning and end of each shift. If any discrepancy is noted Director of Nursing/Executive Director must be immediately notified. Oversee all care provided by caregivers. Confer with the care plan team in the development of the comprehensive care plan. Document resident care problems, abnormal findings, and changes in resident's condition. Report to physician, supervisor, and resident/resident's responsible party. Monitor implementation of preventive measures such as nutrition/hydration measures, and toileting/incontinence care.
