SUMMARY: The School Counselor is responsible for developing treatment-related goals on individualized education program (IEP) and providing trauma-informed counseling services to adolescents (junior/senior high studenst) served by Hammitt Schools. Also provides consultation and crisis response to assist employees in their interventions with students and facilitates training and support for parents.

ESSENTIAL FUNCTIONS:
Assessment and Treatment Planning: Conducts initial and ongoing assessments of students. Develops goals and objectives with each student in accordance with the student's IEP.
Counseling: Provides individual, family, and group counseling in accordance with each student's IEP.
Case Management: Provides case management, mentoring, advocacy, support, and education that will protect children and preserve families.
Crisis prevention and intervention: Provides consultation and support to students, parents, and school employees to assist with crisis prevention and intervention. Provides direct crisis intervention to students as needed.
Parent Education and Support: Provides education and support services to parents.
Requirements
Master's degree in social work, psychology, guidance and counseling, or related field required.
Professionallicensure through ISBE or the Department of Professional Regulation required. School Social Worker licensure preferred.
Knowledge of and skills necessary to apply the principles of adolescent mental health treatment and special education to Hammitt Junior-Senior High School students.

Coordinate with referral sources such as hospitals, law enforcement, schools, and partner agencies
Collaborate with internal teams-crisis clinicians, case managers, psychiatric providers, and leadership-to ensure timely, coordinated support
Document assessments, interventions, and follow-up in the electronic health record

Ability to engage individuals experiencing high-stress, emotionally intense situations
Flexibility-crisis work is dynamic, and each day looks different
Working knowledge of behavioral health diagnoses and treatment approaches
Ability to support individuals with serious mental illness, substance use disorders, or co-occurring conditions
Understanding of psychiatric services, hospital systems, and community resources
Strong decision-making and problem-solving skills
Clear, professional written and verbal communication
Ability to work independently in community settings while collaborating as part of a team
Ability to navigate electronic health record systems
Proficiency in Microsoft Word and Excel
Commitment to professional development and continuous learning
Education & Requirements
Five (5) years of supervised experience in behavioral health
Bachelor's degree in a human services field
Graduate degree in counseling, psychology, social work, or related field preferred
CADC, LSW, or LPC strongly preferred
Must obtain CADC or licensure (LSW, LPC) within 2 years of hire
Minimum 21 years of age
Valid driver's license, reliable transportation, and proof of auto insurance
Prior experience in crisis services strongly preferred
