Position Details
Provide intake services by evaluating incoming referrals to determine the appropriate treatment intervention.
Prepare diagnostic assessments, case histories and other related correspondence as per Merakey, DPW and DOH policies and regulations.
Develop Treatment Plans: Develop a preliminary treatment plan based on client's presenting problem. The preliminary treatment plan is to be completed by the end of the second counseling session. A Master Treatment is to be developed by the end of the fourth counseling session. The therapists and client will identify individualized treatment issues, long and short term goals, and the type and frequency of services to meet those goals. Therapists will review and revise treatment plans with the client every 60 days. The Master Treatment plan and subsequent Treatment plan updates are to be reviewed and signed by the client, therapists, clinical supervisor and psychiatrist or supervisory physician.
Provide ongoing therapy and supportive counseling services to assigned caseload. Conduct and complete client case consults with supervisor and/or other clinical staff as often as needed but at least once every 90 days for D&A clients for the first year, once a year thereafter.

Position Details
Provide intake services by evaluating incoming referrals to determine the appropriate treatment intervention.
Prepare diagnostic assessments, case histories and other related correspondence as per Merakey, DPW and DOH policies and regulations.
Develop Treatment Plans: Develop a preliminary treatment plan based on client's presenting problem. The preliminary treatment plan is to be completed by the end of the second counseling session. A Master Treatment is to be developed by the end of the fourth counseling session. The therapists and client will identify individualized treatment issues, long and short term goals, and the type and frequency of services to meet those goals. Therapists will review and revise treatment plans with the client every 60 days. The Master Treatment plan and subsequent Treatment plan updates are to be reviewed and signed by the client, therapists, clinical supervisor and psychiatrist or supervisory physician.
Provide ongoing therapy and supportive counseling services to assigned caseload. Conduct and complete client case consults with supervisor and/or other clinical staff as often as needed but at least once every 90 days for D&A clients for the first year, once a year thereafter.
