Submit necessary referral forms to team supervisor for approval or assignment in electronic health record (EHR). * Ensure all needed documentation and forms are completed and up-to-date for assigned SCOAP patients. Minimum Qualifications: Bachelor's degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. Per Sedgwick County policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance. The counselor's job role is to provide goal-directed, client centered help to clients with an addiction by educating them on the skills or the mechanics to deal with their addiction. ESSENTIAL POSITION RESPONSIBILITIES: 1. Provides services in order to maintain required productivity/billing standard of 1200 hours per year (Exempt), 70% of hours worked (Non-Exempt) 2. Conducts comprehensive assessments through the use of client interviews, review of records, and conferring with other professionals. 3. Counsels clients' in group or individual sessions to help them to cope with any substance abuse problems. 4. Counsels and helps family members to aid them in dealing with, supporting, and understanding the client. Engagement of family in treatment whenever possible. 5.
