Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
Current CPR certification.

Duties Responsibilities
The Physical Therapist / PT is responsible for the assessment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist / PT will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Physical Therapist / PT must set realistic and achievable goals for their patients, document and record the patient's condition, and educate patients and families in an appropriate physical therapy method.
Develops a plan of care for each physical therapy patient.
Provides skilled physical therapy services / interventions in accordance with physician orders.
Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
Collaborates with all disciplines to plan and evaluate team goals for each patient.

Job Summary and Responsibilities
This is a PRN position
At Deaconess Home Care, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker, you'll engage in a range of exciting responsibilities, including:
Establishing a care plan in accordance with the physician's plan of treatment.
Providing crisis intervention as required.
Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient's ability to improve medically.
Providing patient care education to patients and caregivers.
Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
Working with the patient's caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
Completing and submitting all required documentation in an accurate and timely manner.
Job Requirements
Master's degree from an accredited school approved by the Council on Social Work Education. InIndiana, must also be certified under IC25-23.6-4-3.
Currently licensed to practice in the state serviced.
One year social work experience in a health care setting.
Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Be the Rehab Therapy Tech you always wanted to be
* Support therapists' daily patient care and therapy activities.
* Facilitate effective communication with patients, families, and caregivers.

Qualifications
