JOB SUMMARY
The MentalHealth Case Manager will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mentalhealth needs. The MentalHealth Case Manager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County.
ESSENTIAL FUNCTIONS
Conduct individual assessments, assessing functional strengths, resources and needs.
Develop individualized, person-centered, trauma informed community support plans.
Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support.
Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions.
Coordinate services both internally and externally including county case manager, family/guardian of client, work personnel, medical personnel, etc.
Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services.
Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills.
Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
Provide case management to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc.
May transport clients

The practitioner has a two focused level of responsibility: first, for the care, welfare, treatment, and safety of the clients as individuals and secondly, for the facilitation of a work and a treatment culture that promotes trans-disciplinary team work, professionalism, cultural competency, and adherence to best practice principles and interventions. The Practitioner has responsibility for client specific and program outcomes. Primary duties include assessment, crisis assessment and management, planning, individual counseling, skills teaching, group work, case management, household management, financial eligibility assistance, and on-call.

Manages and over-sees client financial needs, living arrangement services, and monitors medication
Facilitates transition and recovery support services
Adheres to Data Privacy Practices/HIPPA
Communicates, receives, and writes information clearly and accurately
Reads and interprets clinical assessments and findings
Writes crisis/treatment/behavioral plans with differential goals and objectives
Writes progress notes, summaries, and discharges
Monitors and records medication administration activities
Provides objective verbal and written critical incident analysis
Documents essential activities related to licensure, welfare and safety

Participates in the 360 degree performance evaluation process

Qualifications
Bachelor's degree in one of the behavioral sciences from an accredited college or university
4,000 hours of supervised experience in the delivery of services to persons with mental illness; or holds a graduate degree in one of the behavioral sciences or related fields and has at least 2,000 post BA hours of supervised experience in the delivery of services to persons with mental illness
License which may be required based on education and standards determined by educational/degree guidelines
All Practitioners are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner
The agency encourages staff to seek license at the highest standard permitted by his/her education and experience
Ability to conduct functional assessments, develop treatment plans, provide crisis and behavioral management, provide goal directed individual counseling, facilitate groups, teach skills, provide case management, problem-solve, and communicate effectively, orally, and in writing
Demonstration through training and testing within a designated time frame of the following core competency areas: First Aide/CPR, Data Privacy (HIPPA), Non-Violent Crisis Intervention, Medication Monitoring/Administration, Principles and Techniques of Psychiatric Rehabilitation (including Over-All Rehabilitation Goal Setting, Functional Assessment, Skills Teaching and Programming), Empathy/Motivational Training, Working with Persons Experiencing Mania, Hallucinations, and Delusions, and Relapse Prevention Planning

You'll be working in a home-like setting, supporting adults who have experienced significant barriers in living due to symptoms of mental illness and substance use disorders.
MentalHealth Counselor daily support-related responsibilities include spending time interacting with persons served to socialize and build rapport, assisting individuals in meeting daily needs and ensuring ongoing connections with other community supports. You will be a key part of a person's care team, helping to implement treatment.

Duties
Provide courteous and respectful care to all clients.
Spend the majority of each shift interacting with clients, developing relationships and building rapport with clients, and conducting tasks that are directly related to client care

Remain current on client's plans and case notes completed by other staff.

Administer medications according to RSS policy and procedures under the supervision of the facility's nurse.
Inform other staff and Lead Clinical MentalHealth Counselor when quantity of client's medications running low.
Order client medication, as needed.
Update medication sheets when medication changes occur.
Develop weekly menus and shopping list and does grocery shopping for the facility, encouraging participation by clients.
Plan and coordinates recreational/social activities.
Schedule client appointments, as needed.
Plan holiday activities and special events.
Provide input in staff meetings and treatment planning.
Prepare meals, serve and clean up according to Health Department standards.
Assist in training new staff.
Responsible for providing staff coverage and ensuring the security of the facility and general supervision of residents.
Responsible for securing coverage of any missed shifts, planned or unplanned.
Participate in routine cleaning duties.
Perform light house maintenance or request maintenance assistance as needed.
Must be able to transport clients in the community.
Produce and distribute incident reports within (8) hours of the relevant event, as needed.
