Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days.
Initiates and coordinates the plan of care.
Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
Instructs and supervises the patient/family in self-care techniques when appropriate.
Maintains accurate and relevant clinical notes regarding the patient's condition.
Experience Requirements
One year of clinical experience.
License Requirements
Current RN licensure in the state of practice and one year of clinical experience.
Current CPR Certification.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
Additional State Specific Requirements
LA: a minimum two years full time experience working as an RN. However, two years of full time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.

Job Summary
The Personal Care Services Licensed Practical/Vocational Nurse is responsible for: (1) providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which she/he is assigned. Observes, records, and reports (1) signs and symptoms; (2) reactions to treatments and medications; and (3) any change in the patient's physical or emotional condition in a timely matter to the Registered Nurse and physician as applicable.
Participates in the organization's Performance Improvement and Compliance programs and adheres to policies and procedures.

Must meet educational/experience requirements outlined in the laws of licensure states.
A minimum of one (1) year of acute care experience preferred; previous home care experience preferred. Must have current LPN licensure in state of practice.

RESPONSIBILITIES:
Be able to perform welding on various materials at required positions using different welding processes, including, but not limited to, GTAW, SMAW, FCAW, GMAW, and SAW.
Lay out, position, fit, and weld various piping and structural components, including pipes, flanges, fittings, valves, piping supports, structural plates, beams, etc., in accordance with the supplied piping/structural fabrication drawings.
Set up, troubleshoot, and operate welding machines in accordance with job specifications and welding procedures.
Adjust valves, gauges, and flames as needed, and be capable of handling compressed gas and oxygen cylinders safely.
Operate air arc gougers, grinders, and other industrial machines, tools, and equipment.
Participate in JSAs and all other Safety, Health, and Environmental Processes.
Conduct behavior-based safety observations.
QUALIFICATIONS:
3 or more years of prior fabrication experience
Experience using safe work habits
6G - TGAW/FCAW
Candidates will be required to pass a Hands-on Welding Test
