The Palliative Care Nurse Practitioner/APRN provides direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care. Job DescriptionDescription: Attention all Nurse Practitioners, we are calling you to partner with us as independent contractors providing services to our Military Service members. We primarily work weekends, with some occasional weekdays under the Reserve Health Readiness Program (RHRP) in the State of Washington on an "as needed basis". Our events are usually held once a month on the weekends, but occasionally fall on weekdays. It's up to you how frequently you work our events. We provide health readiness services to meet the medical and dental requirements to maintain a deployable military force for the following: U.S. Army Reserve (USAR) Army National Guard (ARNG) U.S. Navy Reserve (USNR) U.S. Marine Forces Reserve (MARFORRES) U.S. Coast Guard Reserve (USCGR) Air National Guard (ANG) U.S. Air Force Reserve (USAFR) Responsibilities: Support Physical Health Assessments of military personnel. Review health history, service member medical records, and medical questionnaires. Interview military personnel to determine current and prior medical concerns. Identify any concerns that need to be addressed immediately, inclusive of Behavioral Health. Document interview results for readiness classification in the service member's medical record using the proprietary system. Purpose The Nurse Practitioner ensures appropriate comprehensive health care to support the clinical care team in achieving Neighborcare's goals for quality care, productivity and patient satisfaction. Perform independent clinical activities, and collaborate with physicians, nurse practitioners, and physician assistants when appropriate to achieve quality patient care. Primary Responsibilities: * Role does not have support of a Medical Assistant, so medical provider will room patients, collect vital signs, administer vaccines/injections, dispense medications, conduct Point of Care testing, perform phlebotomy, clean instruments, and process lab samples. The provider will also be responsible for inventory management of medications, medical supplies, and vaccines. * Provide patient centered preventative care, as well as diagnoses and treatment of acute and chronic conditions * Identify the need for referrals and provide overall care coordination in a team-based setting * Assure accurate and timely completion of all clinical records; Completion of paperwork in a team-based approach * Completing necessary paperwork for patient care such as, prior authorizations and communications with outside specialists * Timely response to communications and test results; Provide feedback to appropriate teams and/or individuals when necessary * Maintain confidentiality of all client information. * Attend required in service/training/staff meetings as scheduled. * Perform other duties as assigned.
