Scope and Purpose for Resident Aide/Caregiver
Will be assigned to provide service to a particular resident/group of residents and will assure that all aspects of the residents' Personal Service Plan are implemented. Must assure the resident is treated with dignity and respect, assuring that each resident remains optimally independent.
Resident Aide/Caregiver Duties and Responsibilities
Responsible to assure residents' daily care and service needs are met, including all basic activities of daily living (bathing, grooming, hygiene, toilet assistance, dressing, eating, socializing, participating in activities, etc.)
Assists residents to participate in Life Enrichment Events - may participate directly through leading an activity, assisting the LEC, or assisting residents to attend.
Follows resident service plan for level of assistance and support needed.
Reporting changes in the resident's condition and needs as they occur.
Light housekeeping chores, (including bed making, linen changes, straightening and dusting, trash removal, laundry, etc.)
Uses safe and proper techniques for chemical/cleaning solutions, as well as storage of products, tools, and carts.
Will practice appropriate safe, sanitary hygiene.
Other duties as assigned or as may be necessary to assure that the goals and objectives of this living center are fulfilled

Documents provision of services on company forms.
Operates designated medical equipment, copy, scan, fax machines, and telephone.
Escorts residents and coordinates internal transportation needs.
Assists residents with personal correspondence/telephone use when requested.
Observes and reports changes in residents' physical condition and cognitive/emotional status to Wellness Director or supervisor, as needed.

Monitors for environmental safety hazards.

Optimal Care Hospice Aides are vital members of the interdisciplinary hospice team, working under the supervision of a Registered Nurse to provide compassionate care that promotes comfort, safety, and dignity for patients.
What You'll Do:
Assist patients with personal care, including bathing, skincare, toileting, and oral hygiene
Support safe mobility and transfers using Hoyer lifts, wheelchairs, gait belts, and assistive devices
Provide companionship and emotional support to patients and caregivers

SUMMARY OF JOB RESPONSIBILITIES
The Hospice Aide is a paraprofessional member of the Interdisciplinary Team who works under the supervision of a RN and performs various services for a patient as necessary to meet the patient's personal needs promote comfort and to enable the patient to remain in the home by supplementing the patient 's/family's/caregiver's abilities for home management. The Hospice Aide is responsible for observing the patient, reporting, and documenting these observations and care performed.
The Hospice Aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
RESPONSIBILITIES AND DUTIES
Responsibilities of the Hospice Aide, but not limited to, the following:
* Performs personal care needs after given a patient's assignment and attending to their requests promptly. If unable to perform a certain task, report to the Hospice Nurse or designee immediately.
* Meets safety needs of patient and using equipment safely and properly (footstools, side rails, O2, etc.)
* Provides or assists patients in all aspects of personal care, including baths, back rubs, oral hygiene, shampoos, changing bed linen as part of the Plan of Care.
* Assists in dressing and undressing and feeding the patient as assigned.
* Plans and prepares nutritious meals and other household tasks, which are part of the Plan of Care.
* Takes and records oral and axillary temperatures, pulse, respiration, and blood pressure when ordered (within the training limitations of the aide).
* Provides proper care and observation of patient 's skin to prevent breakdown of tissue over bony prominence.
* Reports on patient's condition and significant changes to the appropriate Interdisciplinary Team member. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
* Assists in ambulation and exercise as instructed by the Hospice Nurse or Therapist.
* Offers and assists with bedpans and urinals.
* Provides assistance as assigned with light laundry needs.
* Performs range of motion and other simple procedures as an extensional therapy service as ordered (within the training limitations of the Aide).
* Provides respite for patients/families/caregivers when on-site, as appropriate.
* Keeps patient 's living area clean and orderly, as assigned.

JOB LIMITATIONS
The Hospice Aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the Hospice Aide will not administer medications, take the attending physician or designee's orders, or perform procedures requiring the training, knowledge and skill of a licensed nurse, specifically sterile techniques.
