About the Position
We are looking for a Residential Care Associate II to join our team at our Madrone Ridge SRTF in Junction City, Oregon! The Residential Care Associate II will provide support to the residents according to their individualized treatment plan with a focus on personal care services, environmental intervention, and limited group social and recreational treatment activities as assigned by utilizing an approved evidenced-based group curriculum. They will provide limited treatment services and personal care supports for clients that help them develop appropriate skills to increase or maintain their level of functioning. The Residential Care Associate II will administer prescribed medications, follow physician's orders for all medications and/or procedures, document on medical charts and count sheets according to established procedures, and responsibly assist clients with self-administration of medications within agency policies and procedures. They will transport clients to and from the program as necessary to engage and provide treatment services. They will maintain compliance with established productivity standards for documentation of services.
This position falls under the AFSCME bargaining unit. Wages, benefits, and working conditions are set in accordance with the collective bargaining agreement.
Training Program
With our MHACBO-accredited training program, you will earn CEUs towards applicable mental health certifications, while attending your required, paid training, and receiving qualified clinical supervision gaining on-the-job experience.

Thursday through Saturday, 10:00pm - 8:00am (Part Time, Night)

What You'll Need
Must be eligible for Qualified Mental Health Associate registration through the Mental Health & Addiction Certification Board of Oregon (MHACBO). Eligibility requires 1 out of 3 Criteria below:
Bachelor's degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or related field; OR minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting; OR equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training

* Rounds regularly, 3 times per 8 hour shift, to inpatient ICU floors. Collaborates with clinical staff to assess patient needs before each task (lift or reposition), including type of lift to be performed, condition of patient, and state of patient's physical disability. Performs patient transfers or repositioning using the appropriate lifting and transfer equipment. Identifies availability of necessary equipment and supplies for use in transfer or reposition.
* Responds to patient falls and urgent situations in an expedited manner (in less than 10 minutes). Collaborates with clinical staff to assess patient needs before lift or transfer. Identifies availability of necessary equipment and supplies for use in lift or transfer, using patient retrieval systems, such as HoverMatt or HoverJack, when necessary.
* Responds to requests for patient lifts or transfer from on-site and off-site (CHH and Dental School only) clinics (in less than 20 minutes). Collaborates, if possible, with clinical staff on scheduling prior to patient appointment. Responds to requests via Vocera, pagers or through the Transportation Dispatch.
* Participates in MTP's on the highest priority basis. Responds promptly to the Transportation Dispatchers directives. Maintains continual communication with dispatcher using Vocera and face-to-face communication with MTP clinical personnel. To ensure timeliness of this service, the following must be observed: use of elevators is not permissible and the first response must occur within 30 seconds of pager notification.

Report any known or suspected unauthorized attempt to access facility's information system. Perform only those nursing care procedures that you have been trained to do. Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).
Greet residents and escort them to their room. Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. Inventory and mark the resident's personal possessions as instructed. Store resident's clothing.
Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. Transport residents to new rooms or to the receiving area. Assist with loading/unloading residents' to/from vehicles as necessary.
Participate in and receive the nursing report as instructed. Follow established policies concerning exposure to blood/body fluids. Make beds as instructed. Put extra covers on beds as requested. Check residents routinely to ensure that their personal care needs are being met.• Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).
Record the resident's food/fluid intake. Report changes in the resident's eating habits to the Nurse Supervisor/Charge Nurse. Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.).
Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. Follow work assignments, and/or work schedules in completing and performing your assigned tasks. Cooperate with inter departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. The Lift Team assists direct patient care providers using safe patient mobilization techniques such as performing equipment assisted lifts, transfers, turns and repositioning of patients in clinical environment. The Lift Team strives to promote a safe patient handling environment, for both patients and care providers. Lift Team member's coach healthcare clinical staff in use of lift techniques and transfer equipment as needed. This team also participate in Massive Transfusion Protocols (MTP), transporting vital blood products and labs in an expedient and dependable manner.
Function/Duties of Position
Rounds regularly, 3 times per 8 hour shift, to inpatient ICU floors. Collaborates with clinical staff to assess patient needs before each task (lift or reposition), including type of lift to be performed, condition of patient, and state of patient's physical disability. Performs patient transfers or repositioning using the appropriate lifting and transfer equipment. Identifies availability of necessary equipment and supplies for use in transfer or reposition.
Responds to patient falls and urgent situations in an expedited manner (in less than 10 minutes). Collaborates with clinical staff to assess patient needs before lift or transfer. Identifies availability of necessary equipment and supplies for use in lift or transfer, using patient retrieval systems, such as HoverMatt or HoverJack, when necessary.
Responds to requests for patient lifts or transfer from on-site and off-site (CHH and Dental School only) clinics (in less than 20 minutes). Collaborates, if possible, with clinical staff on scheduling prior to patient appointment. Responds to requests via Vocera, pagers or through the Transportation Dispatch.
Participates in MTP's on the highest priority basis. Responds promptly to the Transportation Dispatchers directives. Maintains continual communication with dispatcher using Vocera and face-to-face communication with MTP clinical personnel. To ensure timeliness of this service, the following must be observed: use of elevators is not permissible and the first response must occur within 30 seconds of pager notification.
Transports patients using wheelchair, stretcher, hospital bed, and morgue cart to and from various locations include, but not limited to: nursing units, diagnostics, pre-op, life flight pad and morgue. Transfer patients to and from stretcher, hospital bed, exam table and wheelchair using transportation-assist devices such as Hovermatts, Slipps, slider boards and backboards.
Transports medical equipment, mail, labs, blood products and supplies to and from various locations within the OHSU environment. These locations are, but not limited to: nursing units, diagnostic areas, clinics, exam rooms, operating rooms, laboratories, morgue or offices.
Operates elevators for patient and equipment movement. Relieves elevator attendants for breaks.
Cleans and disinfects transportation medical equipment: stretchers, wheelchairs, morgue carts, HoverMatts, pillows and utility transport carts.
Other duties as assigned.
Required Qualifications
High School diploma or equivalent; must be able to lift 50 pounds or push 300 pounds using appropriate equipment and body mechanics; must be able to follow written and/or verbal instructions; AND
Six months experience in a medical or healthcare environment or 90 days training with patient transportation.
Excellent communication with public and staff.
Strong customer service experience.

This position also includes all duties from the lower levels, Transportation Aide 1 and 2. All transporting and triaging work will be done using the Teletracking transportation software and the current technology used to receive transportation jobs.
Function/Duties of Position
* Transport patients using a wheelchair, stretcher, hospital bed, or morgue cart to and from different hospital locations. These locations include, but are not limited to: nursing units, diagnostic areas, pre-op, the life-flight pad, and the morgue. * Triage (prioritize) urgent patient requests using mobile technology. This includes responding to requests and helping solve problems with different hospital units.

* Transport medical equipment and supplies such as lab materials, blood products, and other items to different OHSU locations. These locations include nursing units, diagnostic areas, clinics, exam rooms, operating rooms, laboratories, the morgue, and waterfront campuses.
* Complete end-of-shift reports, document staffing call-outs, and update leadership when staffing or operational issues come up. This is to help make sure patient care continues smoothly. When needed, lead huddles and hand off information to the next shift.
* Clean and disinfect transportation equipment, including stretchers, wheelchairs, morgue carts, HoverMatts, and utility carts.

Required Qualifications

Function/Duties of Position
Transport patients using a wheelchair, stretcher, hospital bed, or morgue cart to and from different hospital locations. Transfer patients using devices such as HoverMatts, Slipps, slider boards, backboards, and ceiling lifts.
Triage (prioritize) urgent patient requests using mobile technology. This includes responding to requests and helping solve problems with different hospital units.
Assist Health Unit Coordinators (HUCs) and department staff with TeleTracking. Help make sure transport jobs are requested correctly based on where they start and where they need to go.
Transport medical equipment and supplies such as lab materials, blood products, and other items to different OHSU locations. These locations include nursing units, diagnostic areas, clinics, exam rooms, operating rooms, laboratories, the morgue, and waterfront campuses.
Complete end-of-shift reports, document staffing call-outs, and update leadership when staffing or operational issues come up. This is to help make sure patient care continues smoothly. When needed, lead huddles and hand off information to the next shift.
Clean and disinfect transportation equipment, including stretchers, wheelchairs, morgue carts, HoverMatts, and utility carts.
Perform other duties as assigned.
Required Qualifications
High School Diploma or GED.
Must be responsible and properly trained in the transportation of bio-hazardous items within 90 days of hire.

Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director. Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or pharmacist
Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements. Follow the labeling policies and practices of the facility.
Ensure that all medications administered are properly labeled. Follow facility procedures in regard to charting medications. Assist in documenting and removing medications that are discontinued by the attending physician. Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services.
Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you. Follow facility procedures in holding medications for residents who are transferred from the facility orwhen the attending physician discontinues medications Observe and verify that medication is ingested or applied as directed.
Document any instance where prescribed medication is not administered, including reason(s) for refusal. Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents. Verify the identity of the resident before administering the medication treatment.
Accurately measure, record, and report the vital signs of residents. Follow the facility's procedures allowing residents to self administer his or her medications.
