Basic Qualifications

Follow CMOP policies regarding documentation and reports to meet VA and TJC policy.
Note any unusual doses, wrong drug, wrong strength, or drug mix up and report the information in accordance with local CMOP policies.
Provide full checking, labeling, and quality assurance within the guidance (written VHA handbooks, written CMOP policies, and training that will be provided by the CMOP) of the Department of Veterans Affairs regarding pharmaceutical medications.
Dispense medications within corporate and regulatory guidelines for controlled substances.
QUALIFICATIONS:
Shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia.
Shall be Certified Technician (CPhT) with a current valid certification by the National Pharmacy Technician Certification Board (PTCB).
Must be licensed for controlled substances.
Pass a background check (Tier 1 investigation).
EDUCATION:

The annual salary range for this position is $98,000.00 - $172,000.00 *This pay range is for a 40 hours/week schedule. The CMOP program provides prescription mailings for veterans and other patients. Services being sought will be utilized to assist in supporting CMOP's mission by preparing prescriptions and packages in an accurate, timely, and cost-effective manner without disruption in services or production.
The Candidate
ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. The candidate's leadership attributes in Pharmacy management shall ensure the delivery and attainment of patient care quality metrics, evaluate the performance of Pharmacy Technicians duties, and drive client satisfaction outcomes.

In order to perform the duties set forth for this project, the Registered Pharmacist must meet both the minimum education and experience requirements set forth:

Education:
Bachelor of Science in Pharmacy degree that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Licensure:
A current, active, valid, and unrestricted license to practice pharmacy in the State of Tennessee or in possession of an active license from another state that enjoys reciprocity with the state of Tennessee.
The candidate must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE).
Experience:
Demonstrable proficiency in the compounding of prescriptions, drugs, chemicals, and pharmaceutical supplies required in the operation of a pharmacy;
Dispense medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects;
Conduct drug utilization reviews, counsel patients on the proper use of medication while evaluating medication use patterns and outcomes for patients leveraging adherence best practices;
Apply knowledge of state and federal laws and regulations pertaining to the legal operation of a pharmacy to comply with laws, regulations, and policies governing pharmacy operations;
Provide oversight of CMOP facility by developing and implementing operational procedures, maintain accurate records and document actions taken while safeguarding the confidentiality of medical records;

The CMOP program provides prescription mailings for veterans and other patients. Services being sought will be utilized to assist in supporting CMOP's mission by preparing prescriptions and packages in an accurate, timely, and cost-effective manner without disruption in services or production.
The Candidate
ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. The candidate's leadership attributes in Pharmacy management shall ensure the delivery and attainment of patient care quality metrics, evaluate the performance of Pharmacy Technicians duties, and drive client satisfaction outcomes.

Qualifications
Education:
Bachelor of Science in Pharmacy degree that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Licensure:
A current, active, valid, and unrestricted license to practice pharmacy in the State of Tennessee or in possession of an active license from another state that enjoys reciprocity with the state of Tennessee.
The candidate must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE).
Experience:
Demonstrable proficiency in the compounding of prescriptions, drugs, chemicals, and pharmaceutical supplies required in the operation of a pharmacy;
Dispense medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects;
Conduct drug utilization reviews, counsel patients on the proper use of medication while evaluating medication use patterns and outcomes for patients leveraging adherence best practices;
Apply knowledge of state and federal laws and regulations pertaining to the legal operation of a pharmacy to comply with laws, regulations, and policies governing pharmacy operations;
Provide oversight of CMOP facility by developing and implementing operational procedures, maintain accurate records and document actions taken while safeguarding the confidentiality of medical records;
Apply inventory control expertise in establishing medication-handling procedures for the storage and preservation of medications;

Job Summary:
Reviews medication orders issued by physicians or other authorized prescriber to assure accuracy and determine formulas and ingredients needed, assuring that all orders are filled and labeled (including secondary labels) properly. Performs clinical functions as outlined by the Pharmacist in Charge. Maintains the integrity and accuracy of the drug distribution and delivery systems. Ensures the appropriate and safe utilization of high-risk medications and controlled substances as outlined by department and hospital policies. SCOPE OF WORK
Contract providers shall coordinate and\/or provide clinical Pharmacy services that involve complex problems of a highly specialized nature. Serve as a liaison between the Department of Pharmacy Medical Staff, Department of Nursing and patients for pharmaceutical matters and as an expert on pharmaceuticals, pharmacokinetics and pharmacotheraputics.
Contract Clinical Pharmacist shall serve as a therapeutic consultant, attend medical rounds and conferences with the physicians to provide information used in evaluation and implementation of optimal medication plans for inpatients and outpatients.
Performs a comprehensive review of patient's medical history.
Analyze selected medication programs for accuracy, incompatibilities, contraindications, and appropriate doses. Confers with the physician for required monitoring procedures, and laboratory test.
Monitors patient medication therapy for effectiveness, side effects, and adverse reactions. Recommends alternate therapy to the physician when appropriate.
With approved clinical privileges, initiates orders for selected drug therapies and diagnostic tests according to established protocols.
Develops, orders, and adjust individualized medication regimens per protocol or consent of physician.
Provides medication information to patients, evaluates the likelihood of patient medication adherence and provides suggestions to the patient and the physician for improvement of the therapy regimen.
Counsels patients on the use, proper method for administration of medications, the common potential side effects from medication usage, and precautions and other general information.
Establishes and maintains a comprehensive pharmaceutical dosing service.
Develops individual dosing regimens based on pharmacokinetics of drugs employed, disease states, and medication history of the patient.
Monitors selected medication levels and disease physiology for optimal pharmacokinetic dosing.
Contract Pharmacist shall adjust medication per established guidance or provides recommendations to the physician related to possible changes in dose, interval of medication administration and appropriate times to draw laboratory samples. Coordinates with the Pharmacy to ensure optimal provision of medication therapy.
Contract Pharmacist shall assists the medical staff in designing, coordinating and accomplishing an effective medication utilization evaluation (MUE) program to promote and sustain optimal therapeutic effectiveness and reduce waste. Conducts studies and audits of drug use. Initiates cost effectiveness analysis, reviews specific drug use and total drug therapy for high cost\/ high volume disease states. Makes recommendations to the physician and to the Pharmacy and Therapeutics committee for effective utilization of resources.
Contract Pharmacist shall perform medication distribution functions to include the filling, checking, counseling and dispensing of outpatient prescriptions, preparation of intravenous admixture and filling, checking and dispensing of unit dose medications.
