Key Duties and Responsibilities: Obtain home medication histories by interviewing the patient/family/significant other for patients arriving through the Emergency Department to be admitted to the inpatient units. Utilize the external prescription history, patient's electronic medical record (EMR), and the patient's pharmacy to verify the correct medication history. This includes but is not limited to prescription, non prescription, herbals, and supplements. Compile all of the information obtained to determine a complete medication history including the medication name, dose, frequency, and adherence. Document the correct medication history in the EMR. Positively interacts with patients, other hospital guests, and hospital staff to enhance the quality of service provided. Minimum Requirements: High School Diploma or equivalent CT Pharmacy Technician registration. CPhT is preferred but not required. 1 - 2 years of retail or medication reconciliation experience preferred An appropriate level of understating and training in medical terminology and medication related terms. Current/fluent in pharmacy terminology, medications and dosages. Proficient with using a computer, typing, and organization. Required to concentrate for extended periods of time, provide high output of work on short notice, establish priorities, make quick accurate decisions. Preferred Skills: Be able to prioritize competing demands, work independently, and show flexibility and teamwork. Must demonstrate excellent communication skills to patients and team members.
