Job Description

of the Operations Supervisor, this position shall perform phlebotomy on patients.
Duties and Responsibilities:
1. Perform phlebotomy professionally, competently and with compassion for the patient.
2. Maintain a safe and clean environment for all staff members and patients, including without limitation,
collecting and disposing of specimens and other contaminated material in
accordance with established policies and procedures and with adherence to the
appropriate standards of infection control and body fluid precautions and
protective equipment when necessary.
3. Ensure that requisitions are completed properly, and the patient demographic, diagnosis, insurance and
physician information are included on or attached to the requisitions. Diagnostic information shall be in a
narrative or ICD-9 code format and valid only if legible.
4. If a patient is a Medicare patient, you must review the National Coverage Decisions (“NCD”) and Local
Medical review Policies (“LMRP”) for the tests ordered on the requisition form
to determine if an Advance Beneficiary Notice (“ABN”) must be completed. If the form is needed, correctly complete the
form with the patient.
5. Ensure that the Medicare Secondary Form (“MSP”) is completed when necessary.
6. Accurately collect all specimens necessary to fulfill the clients' lab orders.
7. Provide accurate communication to clients and their staff regarding the proper ordering of lab
tests.
8. Ensure that draw station where you work is kept clean and adequately stocked with supplies every day.
9. On a daily basis, inform your Operations Supervisor of your job performance issues, concerns and schedule.
10. If you work for UWL and are located within a physician's office, you are not permitted to do any work for
the physician and/or the physician's medical practice, your sole responsibility is to collect specimens to be sent to a UWL client's laboratory.
Additional Responsibilities:
1. Perform the duties of a courier if called upon.
2. Deliver supplies when necessary.
3. Obtain any additional information if called upon.
4. Contact Medicare patients to fulfill compliance requirements in regard to MSP forms.
5. Any other reasonable task you may be asked to perform by the Operations Supervisor and which is not
included herein.
