Job Description All phlebotomists shall perform their duties in alignment with the vision and values of United WestLabs, Inc. The person doing this job is responsible for a variety of tasks that lead to a high level of quality of customer care and satisfaction in a cost effective manner. Under the direction of the Operations Supervisor, this position shall perform phlebotomy on patients. Duties and Responsibilities: 1. Perform phlebotomy professionally, competently and with compassion for the patient. 2. If a patient is a Medicare patient, you must review the National Coverage Decisions (“NCD”) and Local Medical review Policies (“LMRP”) for the tests ordered on the requisition form to determine if an Advance Beneficiary Notice (“ABN”) must be completed. If the form is needed, correctly complete the form with the patient. 5. Ensure that the Medicare Secondary Form (“MSP”) is completed when necessary. 6. Accurately collect all specimens necessary to fulfill the clients' lab orders. 7. Provide accurate communication to clients and their staff regarding the proper ordering of lab tests. 8. Ensure that draw station where you work is kept clean and adequately stocked with supplies every day. 9. On a daily basis, inform your Operations Supervisor of your job performance issues, concerns and schedule. 10. If you work for UWL and are located within a physician's office, you are not permitted to do any work for the physician and/or the physician's medical practice, your sole responsibility is to collect specimens to be sent to a UWL client's laboratory.
