4. Record evaluations, daily treatment notes, weekly progress notes, 14 day progress report to the physician, and discharge summaries per policy and procedure.
5. Instruct patient/resident's families or nursing staff in maintenance program.
6. Participate in discharge planning.
7. Secure necessary durable medical equipment for patients/residents to facilitate independence in mobility: i.e. orthoses, walkers, canes, elevated toilet seats, and grab bars.
8. Submits logs and all billing information on a timely basis.
9. Reports any problem areas/equipment within each facility to the appropriate supervisor immediately.
10. Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines.
11. May perform other duties as assigned.

Responsibilities:
Initiate and Maintain Therapeutic Programs: Develop and implement personalized treatment plans to help patients achieve their goals.
Conduct Evaluations: Administer initial evaluations, identify primary impairments, and use appropriate outcome measures to create effective treatment plans.
Implement Treatment Programs: Provide therapy, vestibular rehabilitation, home evaluations with recommendations, orthotic and assistive device recommendations, and therapeutic home programs. Engage patients in various mobility exercises, neurodevelopment sequence exercises, flexibility exercises, and extremity manipulations. Assist patients with brain injuries in navigating different surfaces, including hills, stairs, and uneven terrain, providing all levels of assistance as needed.
Provide Pain Relief: Use modalities such as hot packs, cold packs, "Omni stim" units, neuromuscular stimulation units, transcutaneous nerve stimulation units, and perform light and deep massage to relieve pain and improve patient comfort.
Program Management: Continuously check on patient progress to ensure the effectiveness of treatment programs. Review and update patient programs through interim evaluations.
Supervise and Guide: Instruct clinic aides on daily treatment plans, supervise their work, and provide guidance and advice as needed. Supervise students and share your expertise and knowledge.
Reporting and Communication: Maintain accurate records and reports. Consult with patients, insurance representatives, families, and therapists from various clinic disciplines to provide guidance and information.
Qualifications:
Master's Degree or Doctorate in Physical Therapy.
Current licensure or eligibility for licensure in the state of employment.
Membership in APTA is desirable.
The salary for this role is determined by various factors, including your skills, experience, education, training, licensure, and certifications, as well as our business and organizational needs. While it's uncommon for someone to start at the top of the salary range, each compensation decision is unique. Currently, the estimated salary for this position is between $85,000 and $105,000 per year.

Responsibilities
• Position is in an acute inpatient care setting
• Covers Medical/Surgical, ICU, and ER patient areas
• Typical patient population includes general medical, deconditioned, total knee and hip arthroplasty, spinal surgery, general orthopedic, and cerebrovascular accident cases
• Treats approximately 12 patients per day
• Uses Cerner for EMR documentation
• Day shifts primarily 8:00 am - 4:30 pm, with occasional 10:00 am - 6:30 pm
• Weekend coverage required once every third weekend, with Monday and Friday off during that week
• First to float as needed between areas
• Responsible for evaluation and treatment of diverse patient populations in an acute care hospital
• Works alongside one aide to support the caseload; no PTAs available in the department
• Department-specific orientation provided, including familiarization with each area
• Burgundy polo and scrub or khaki bottoms required as dress code
• No holiday coverage required
• Candidates who have been employed at any CommonSpirit, CHI, or Dignity facility within the past year are not eligible
• Local candidates may be considered; specific radius rules may apply based on state

• Active California PT license required; license required if applicable

• Current BLS certification (AHA) required
