Responsibilities: This is a M-F position with traditional work hours doing home visits 4 days a week in the Inland Empire Area and working remotely 1 day a week.
1. Conduct in-home assessments on Alignment members, medication review and health screenings
2. Provide patient education by matching care desired with best care given
3. Identify diagnoses to be assessed in care management and active medical management
4. Partner and communicate with Regional Medical Officer and other team members to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.
5. Coordinate care with multiple stakeholders, including but not limited to PCPs, specialists, and ancillary providers.
6. Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators

Job Description:
We are seeking a PhysicianAssistant for our Century City office at Los Angeles Cancer Network. • Provides HIV, TB and other infectious disease care in accordance with standard medical proactive protocols.
• Provide medical services, within scope of practice as determined by licensure and/or certification. Provide additional medical services, as determined by Chief Medical Officer, within your scope of practice, such as Medication Assisted Therapy (MAT) or Pre-Exposure Prophylaxis (PrEP).
• Accurately complete all necessary charting according to clinic protocols and mandated by funding source guidelines and standard medical practice.
• Records medical history and provides physical examinations and follow-up care.
• Provides individual patient instruction in accordance with the prescribed treatment plan. Works closely with HIV Case Managers in order to provide appropriate education to the patients.
• Prescribes appropriate medication to patients as needed according to medical protocols and standard medical practice.
• Identify physical, nutritional and/or lifestyle problems and refers to appropriate on-site and off-site resources.
• Identify psychosocial and emotional problems/issues and refers to on-site or off-site resources.
• Must be in compliance with all clinical and administrative quality measures such as HEDIS metrics and patient productivity.
• Consults with supervising/collaborating physician or Chief Medical Officer for patient evaluation as needed.

• Serves as a preceptor instructor to Nurse Practitioner, PhysicianAssistant, or medical students as assigned.
• Attends conferences and seminars when requested and will comply with continuing education requirements.
POSITION REQUIREMENTS AND QUALIFICATIONS:
• Satisfactory completion of an accredited Advanced Practice Provider training program.
• Current license in the State of California as a Nurse Practitioner/PhysicianAssistant.
• Completion of additional continuing training in HIV Medicine and Substance Use Disorder.
• Current DEA certification.
• Current Board Certification from accredited organization for the certification of Advanced Practice Providers.
• At least 1 year experience working with HIV+ patients.
• Current CPR/BLS certifications.
• Must relate well to all cultural, ethnic, and socio-economic groups within the East Valley service area.
