Responsibilities: PRIMARY Ability to provide quality care in both clinic and telehealth and meet patient volume goals targeted for tele and in patient as determined. Ability to meet patient volume goals targeted for tele and in-patient as determined. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis. As well as build and maintain confidence and credibility with all employees. Implement clinical and Telehealth protocols as outlined CMO and Supervising Physicians. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings. Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up. Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions. Attest and follow clinical practice guidelines by the Office of Medical Affairs. Attend Mandatory monthly meetings with Supervising physicians and E/M training. Ability to perform responsibilities within standard NP/PA protocols. Teaching: * Provide student instruction and performance evaluation using evidence-based teaching methods and non-punitive feedback. * Provide academic council and remediation to students as needed. * Knowledgeable in course content and effective in teaching assigned subjects. * Serve as a course instructor in appropriate areas. * Ensure the quantity and quality of supplies and equipment are maintained in optimal condition for instruction. * Maintain current standards of practice in all courses taught. * Provide remediation to students as needed. * Maintain appropriate office hours. Practice: * Contribute toward professional growth through workshops, conferences, cross-training, faculty meetings, professional memberships, maintaining certifications and licensure. * Complete continuing medical education required by state licensing board and national certification commission. * Attend and participate in continuing education conferences/workshops. * Participate in state and local PA associations. Advocacy and leadership are encouraged. * Maintain clinical skills through practice and/or scholarly activities. * Coordinate and/or participate in special projects as required. Job Skill Requisites: * Faculty instructors can be the following: * a) NCCPA certified Physician Assistants (with Masters degree in PA), or * b) physicians who are specialty board certified in their area of instruction, or * c) other licensed health care providers qualified in their area of instruction. * If Physician, currently licensed allopathic or osteopathic physician. * If Physician, certified by an ABMS- or AOA-approved specialty board. * Knowledgeable in the education of Physician Assistant Providers, including professional, legislative, and regulatory issues regarding Physician Assistants. * Qualified through academic preparation and/or experience to teach assigned subjects. * Faculty actively serving as supervised clinical practice experience preceptors must hold a valid license to practice at the clinical site. * Minimum of two years occupational experience. Responsibilities: About the Role During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols Administer and order point of care and lab-based testing as needed Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities Benefits of Working at Matrix Flexibility - Visits are scheduled based on YOUR availability.
